摘　　要：目的:探讨应用FOCUS-PDCA程序对皮瓣移植患者心理状态和自我效能的影响。方法:将2017年1月-2018年6月收治的44例患者设为对照组,落实常规护理;将2018年7月-2019年6月收治的41例设为观察组,运用FOCUS-PDCA程序实施干预。干预前、后分别采用焦虑自评量表(Self-rating anxiety scale,SAS)、抑郁自评量表(Self-rating depression scale,SDS)和一般自我效能感量表(General self efficacy scale,GSES)进行评价并比较两组患者的皮瓣成活率、血管危象发生率。结果:干预前两组患者焦虑、抑郁及自我效能评分比较差异均无统计学意义(P>0.05);干预2周、4周后观察组患者的焦虑、抑郁评分低于对照组,自我效能评分高于对照组,差异均有统计学意义(P<0.05);观察组皮瓣成活率高于对照组,血管危象发生率低于对照组,差异均有统计学意义(P<0.05)。结论:应用FOCUS-PDCA程序可有效查找影响患者心理状态的因素,有针对性改进干预措施,可有效改善患者心理状态,提高患者自我效能,降低并发症发生,促进康复。Objective To explore the influence of FOCUS-PDCA program on the mental state and self-efficacy of flap transplant patients.Methods 44 patients admitted from January 2017 to June 2018 were set as the control group to implement routine nursing.41 patients admitted from July 2018 to June 2019 were set as the observation group to implement the FOCUS-PDCA program.Before and after the intervention,Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS)and General Self-efficacy Scale(GSES)were used to evaluate and compare the flap survival rate and vascular crisis rate of the two groups.Results There was no signifi cant difference in anxiety,depression and self-efficacy between the two groups before intervention(P>0.05).After 2 weeks and 4 weeks of intervention,the scores of anxiety and depression in the observation group were lower than those in the control group,and the scores of self-efficacy were higher than those in the control group,the differences were statistically significant(P<0.05).The survival rate of flap in the observation group was higher than the control group,and the incidence of vascular crisis in the observation group was lower than the control group(P<0.05).Conclusion FOCUS-PDCA program can effectively find out the factors that affect the psychological state of patients,improve the intervention measures,improve the psychological state of patients,improve the self-efficacy of patients.It is benefi cial to reduce complications and promote rehabilitation.
