早期锻炼护理模式对先天性并指患者分指术后恢复效果的影响

摘　　要：目的:探究早期锻炼护理模式对先天性并指患者分指术后恢复效果的影响。方法:选取笔者医院收治的先天性并指患者51例,按照入组奇偶数分为对照组和观察组,分别26例和25例。两组患者均进行并指分指术,术后予以抗感染、镇痛等常规护理手段,观察组在此基础上进行早期锻炼护理。观察并分析两组患者炎症因子水平、创面评分及心理评分,随访1年内并发症发生情况。结果:护理干预后,两组患者肉芽组织评分、分泌物评分、汉密尔顿焦虑评分(Hamilton anxiety scale,HAMA)、汉密尔顿抑郁评分(Hamilton depression scale,HAMD)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均降低,且观察组显著低于对照组,差异具有统计学意义(P<0.05)。随访1年,两组患者挛缩、肤色差异、指蹼爬升、指蹼宽度深度不满意等发生情况比较差异均无统计学意义(P>0.05)。结论:先天性并指患者分指术后在常规护理基础上进行早期锻炼,对并发症发生情况无明显影响,但对创面愈合的促进作用和对负性心理的改善作用更强。Objective To explore the effect of early exercise nursing mode on postoperative recovery of congenital syndactyly patients.Methods 51 patients with congenital syndactyly admitted to our hospital were selected and divided into the control group and the observation group according to the odd and even number of the patients,with 26 cases and 25 cases respectively.Both groups of patients underwent syndactyly and finger separation,and received conventional nursing methods such as antiinfection and analgesia after operation.The observation group received early exercise nursing on this basis.The level of inflammatory,the scores of wound and psychological,and complications occurred within one year of follow-up were observed and analyzed between the two groups.Results After nursing intervention,the scores of granulation tissue,secretion,Hamilton anxiety scale(HAMA),Hamilton depression scale(HAMD),and the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)in the two groups were all decreased,and those in the observation group were signifi cantly lower than those in the control group,the differences were statistically signifi cant(P<0.05).Following up for 1 year,there was no statistical difference in the number of patients with contracture,skin color difference,fin climbing and fin width and depth dissatisfaction between the two groups(P>0.05).Conclusion Early exercise on the basis of routine nursing for patients with congenital syndactyly after finger separation has no obvious effect on the occurrence of complications,but it has stronger promoting effect on wound healing and improving negative psychology.
