摘 要:目的:探究呼吸康复训练联合百令胶囊通过人RAS同源基因家族成员A(RhoA)/Rho-kinase信号通路对慢性阻塞性肺疾病(COPD)患者的疗效及肺功能的影响。方法:选择2016年2月至2017年9月陆军第八十一集团军医院100例COPD患者,按照随机数字表法分为对照组和联用组,每组50例,同期挑选50例健康人群作为空白组。对照组患者给予百令胶囊进行治疗,百令胶囊+呼吸康复训练联用组采用呼吸康复训练联合百令胶囊进行治疗。分别分析各组患者肺功能[最大深吸气后第1秒用力呼出气体容积(FEV 1)、用力肺活量(FVC)、FEV 1/FVC]情况和临床疗效,通过免疫印迹、实时荧光定量聚合酶链式反应(qRT-PCR)检测RhoA、RockⅠ/Ⅱ蛋白和mRNA的表达。结果:经治疗,联用组的肺功能各项指标FEV 1、FVC、FEV 1/FVC显著高于对照组[(2.34±0.91)L比(1.91±0.83)L、(2.98±0.83)L比(2.34±0.86)L、(79.63±9.95)%比(76.13±6.97)%](P<0.05);与空白组相比,其余各组RhoA mRNA、RockⅠ/ⅡmRNA显著增高(P<0.05),与对照组相比,联用组RhoA mRNA、RockⅠ/ⅡmRNA显著降低(2.46±0.18比4.38±0.21、1.72±0.11比2.36±0.24、1.79±0.24比3.34±0.21)(P<0.05);经治疗后,联用组6 min步行实验(6MWD)和临床总有效率[92.00%(46/50)比78.00%(39/50)]显著优于对照组(P<0.05)。结论:呼吸康复训练与百令胶囊联用能够显著改善COPD患者肺功能,提高临床疗效,并且可以通过RhoA/Rho-kinase信号通路下调RhoA、RockⅠ/Ⅱ蛋白和mRNA的表达。Objective:To investigate the effect of respiratory rehabilitation training combined with bailing capsule on the efficacy and lung function of patients with chronic obstructive pulmonary disease(COPD)through RhoA/Rho-kinase signaling pathway.Methods:One hundred patients with COPD from February 2016 to September 2017 in the Eighty-Eleventh Army Hospital of Hebei Army were randomly divided into control group(50 cases)and bailing capsule+respiratory rehabilitation training group(50 cases).At the same time,50 healthy people were selected as blank group.The patients in the control group were treated with bailing capsule,and patients in the combined treatment group were treated with respiratory rehabilitation training combined with bailing capsule.The pulmonary function(FEV 1,FVC,FEV 1/FVC%)and clinical efficacy were analyzed.The expression of RhoA,RockⅠ/Ⅱprotein and mRNA were detected by immunoblotting and qRT-PCR.Results:After treatment,the indexes of pulmonary function in the combined group were significantly higher than those in the control group:(2.34±0.91)L vs.(1.91±0.83)L,(2.98±0.83)L vs.(2.34±0.86)L,(79.63±9.95)%vs.(76.13±6.97)%(P<0.05).Compared with the blank group,RhoA mRNA and RockⅠ/ⅡmRNA in the combined treatment groups were significantly higher than those in the control group(P<0.05),and RhoA mRNA and RockⅠ/ⅡmRNA in the combined group were significantly lower than those in the control group(P<0.05).Compared with the blank group,the RhoA mRNA and RockⅠ/Ⅱproteins combined treatment groups were significantly higher than those in the control group(P<0.05),and the RhoA mRNA and RockⅠ/Ⅱproteins in the combined group were significantly lower than those in the control group(2.46±0.18 vs.4.38±0.21,1.72±0.11 vs.2.36±0.24,1.79±0.24 vs.3.34±0.21)(P<0.05).After treatment,the 6MWD and the total clinical effective rate in the combined group were significantly better than those in the control group:92.00%(46/50)vs.78.00%(39/50)(P<0.05).Conclusions:Respiratory rehabilitation training combined
