摘 要:目的了解临汾地区乙型肝炎病毒(HBV)基因型分布,并探讨基因型与血清学指标乙肝病毒表面抗原(HBsAg)、乙型肝炎病毒e抗原(HBeAg),生化指标丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、单胺氧化酶(MAO)及HBV-DNA载量的关系。方法抽取2013年1月至2014年12月在我院就诊的慢性乙型肝炎(CHB)患者272例。采用聚合酶链反应(PCR)荧光探针法和实时荧光定量法检测HBV基因型和HBV-DNA载量;采用化学发光法检测血清HBsAg和HBeAg;采用连续监测法检测ALT、AST、MAO含量。结果 272例CHB患者B基因型占16例(5.9%),C基因型占256例(94.1%),无B型+C型及其他型;HBeAg阳性率与HBV基因型分布差异具有统计学意义(χ^2=12.14,P<0.05);HBV C基因型患者血清学指标HBsAg,生化指标ALT、AST、MAO及HBV-DNA载量高于B基因型(t=13.34,P<0.05;t=12.82,P<0.05;t=15.67,P<0.05;t=24.51,P<0.05;t=13.83,P<0.05);HBeAg阳性患者HBV-DNA与HBsAg和MAO存在较强的相关性,相关系数分别为0.75和0.79。结论临汾市HBV基因型以C型为主,C基因型患者HBeAg阳性率、HBsAg、ALT、AST、MAO含量及HBV-DNA载量均高于B型。Objective To investigate hepatitis B virus(HBV)genotypes distribution and the relationship between HBV genotypes and the level of serological markers,biochemical markers, hepatitis B virus-deoxyribonucleic acid(HBVDNA). Methods Two Humdred and seventy two cases with chronic hepatitis B(CHB)patients were chosen from patients in our hospital. HBV genotypes and HBV-DNA quantitation were tested applying polymerase chain reaction(PCR)fluorescent probe and real-time fluorescent quantitative PCR;hepatitis B surface antigen(HBsAg) and hepatitis B e antigen(HBeAg) were detected by chemiluminescent assay;ALT aspartate transaminase(AST) and monoamine oxidase(MAO)were teste by continuous monitoring method. Results Of the 272 cases,genotype B accounted for 16(5.9%),genotype C accounted for 256(94.1%),no B+C genotypes and other genotypes;There were significant differences between HBV genotypes and HBeAg positive rate(χ^2=12.14,P<0.05);The level of serological markers HBsAg,biochemical markers ALT,AST,MAO and HBV-DNA of C genotypes were more higher than those of genotypes B(t=13.34,P<0.05;t=12.82,P<0.05;t=15.67,P<0.05;t=24.51,P<0.05;t=13.83,P<0.05);There were stronger correlation between HBVDNA and HBsAg(r=0.75),MAO(r=0.79)in HBeAg positive patients. Conclusion In Linfen area,HBV genotype C was dominating,HBeAg positive rate,the level of HBsAg,ALT,AST,MAO and HBV-DNA of genotype C were higher than those of genotype B.
