摘 要:目的研究白细胞介素-1β(IL-1β)水平与多发伤患者损伤严重程度及全身炎症反应综合征(SIRS)、多器官功能障碍综合征(MODS)之间的关系。方法收集2018年6—11月北京市平谷区医院急诊科接诊121例多发伤患者的临床资料,按结局分为单纯多发伤组、SIRS/MODS组和死亡组,比较3组患者资料和IL-1β水平变化情况,分析IL-1β峰值与损伤严重程度评分(injury severity score,ISS)相关性。结果死亡组的ISS评分(P=0.002)及血清IL-1β水平(P=0.013)、住院天数(P=0.028)等显著高于SIRS/MODS组和单纯多发伤组。患者血清IL-1β水平于入院后第1天达峰值,IL-1β峰值水平与患者入院时ISS评分呈正相关(r=0.696,P=0.000)。结论IL-1β水平与多发伤患者损伤ISS评分存在正相关,可作为预测多发伤患者临床结局的潜在指标。Objective To study the relationship between the level of IL-1β and the severity of injury in multiple trauma patients and Systemic Inflammatory Response Syndrome(SIRS),Multiple Organ Dysfunction Syndrome(MODS).Methods The clinical indexes of 121 patients with multiple trauma from June 2018 to November 2018 was collected.According to the different outcomes,the patients were divided into three groups:the simple multiple trauma group,the SIRS or MODS group,and the death group.The differences in the clinical parameters and the diversification of IL-1β levels were contrasted.The correlation between the peak levels of serum IL-1β and injury severity scores(ISS)were analyzed.Results The ISS score(P=0.002),serum IL-1β level(P=0.013)and hospitalization days(P=0.028)of the death group were significantly higher than those of the SIRS or MODS group and the simple multiple injury group.The peak level of serum IL-1β was in the first day after admission,and IL-1β peak level was positively correlated with ISS score at admission(r=0.696,P=0.000).Conclusion There is a positive correlation between IL-1β level and ISS score in the patients with multiple trauma.IL-1β can be used as a potential indicator for predicting outcome of patients.
