分类安排医嘱标签在降低静脉药物调配中心摆药差错的效果分析

摘　　要：目的:分析静脉药物调配中心(PIVAS)出现摆药差错的情况及原因,对各工作环节进行梳理,制定相应标签安排及摆药复核措施,降低摆药差错率,提高PIVAS药品调配质量。方法:比较2016年未实行分类安排医嘱标签出现的摆药差错和2017年分类安排医嘱标签出现的摆药差错情况,分析改进前后的摆药差错率差异,评价相关措施的效果。结果:实施后摆药差错率由0.032‰下降到0.004‰,降幅达86.67%。摆药人员由原来的8人减到4人。结论:分类安排医嘱标签,既减少了摆药人员的工作压力,有效减少了摆药差错发生率,保障了药品调配质量,节约了人力成本,提高了PIVAS工作效率。Objective:To analyze the current status and causes of drug dispensing errors in pharmacy intravenous admixture services(PIVAS),sort out all work links,formulate corresponding measures of label arrangements and drug dispensing review,so as to reduce drug dispensing error rate and improve drug dispensing quality of PIVAS.Methods:The errors in the medical order labels without classification from in 2016 and the errors in the medical order labels with classification in 2017 were compared.Effects of corresponding measures were evaluated by reducing the error rate of drug dispensing before and after improvement.Results:After the implementation of classification of medical order labels,the error rate of drug dispensing dropped from 0.032‰to 0.004‰,with a decrease of 86.67%.The number of drug dispensing personnel decreased from 8 to 4.Conclusion:Classification of medical order labels not only reduces the work pressure of drug dispensing personnel,effectively reduces the incidence of drug dispensing errors,but ensures the quality of drug dispensing,saves labor costs,and improves the work efficiency of PIVAS.
