摘 要:为了探讨秦皇岛市儿童EB病毒(EBV)感染的特点和病毒检测意义,收集2017年1月至2019年1月期间本院儿科门诊部和住院部收治的疑诊EB病毒感染患儿541例,采用酶联免疫吸附试验(Enzyme linked immunosorbent assay,ELISA)测定血清EB病毒衣壳抗原Ig M(EB virus capsid antigen Ig M,EBVCA-IgM),使用实时荧光定量RT-PCR测定EBV-DNA拷贝量。收集患儿性别、年龄、合并症、临床症状、血常规、异型淋巴细胞、肝功能、心肌酶谱、血沉、C-反应蛋白、T淋巴细胞亚群和EBV-DNA拷贝量。秦皇岛市儿童EBV感染可见发热、淋巴结肿大、肝脾肿大、咽峡炎、异型淋巴细胞>10%和肝肾功能异常等。儿童EBV感染EBVCA-IgM和EBV-DNA阳性率分别为17.93%(97/541),21.63%(117/541),以学龄前期、夏季及异型淋巴细胞>10%的EBVCA-IgM、EBVDNA阳性率较高,P<0.05。学龄前期是秦皇岛市儿童EBV感染高发时间段,夏季是EBV感染高发季,异型淋巴细胞增加、EBVCA-IgM和EBV-DNA在EBV感染的诊断中具有重要的意义。To explore the characteristics of EB virus(EBV)infection children and significance of virus detection in Qinhuangdao city,541 children with suspected EBV infection admitted to pediatric outpatient and inpatient department of the author’s hospital from January 2017 to January 2019 were selected,serum EB virus capsid antigen Ig M(EBVCA-IgM)was detected by enzyme linked immunosorbent assay(ELISA),EBV-DNA copies was detected by reverse transcription-polymerase chain reaction(RT-PCR).Gender,age,complications,clinical symptoms,blood routine,lymphocyte atypia,liver function,myocardial enzyme profile,erythrocyte sedimentation rate,C-reactive protein,T lymphocyte subsets,and EBV-DNA copies were collected.Fever,lymph node enlargement,hepatosplenomegaly,angina,atypical lymphocyte>10%and hepatorenal dysfunction were observed in children with EBV infection in Qinhuangdao city.The positive rates of EBVCA-IgM and EBV-DNA in children with EBV infection were respectively 17.93%(97/541)and 21.63%(117/541),which were higher in pre-early school age,summer and atypical lymphocyte>10%,P<0.05.Pre-early school age is high-risk period and summer is high-risk season for EBV infection children in Qinhuangdao city.The increasing of atypical lymphocytes,EBVCA-IgM and EBV-DNA have great significance in the diagnosis of EBV infection.
