参附注射液对脓毒症休克患者炎症标志物影响的Meta分析

摘　　要：目的系统评价参附注射液对脓毒症休克患者炎症标志物的影响。方法检索2020年4月12日之前中国知网(CNKI)、万方数据(WanFang)、中国生物医学文献数据库(CBM)、维普中文科技期刊(VIP)、Pubmed、Embase、The Cochrane Library等数据库中关于参附注射液治疗脓毒症休克的随机对照研究(RCT),运用RevMan 5.3软件进行分析。结果纳入12篇RCT,共920例患者。试验组(参附组)脓毒症休克患者肿瘤坏死因子-α(TNF-α)、血清降钙素原(PCT)、C反应蛋白(CRP)、白细胞介素-1(IL-1)、白细胞介素-6(IL-6)及白细胞介素-10(IL-10)均明显低于对照组(常规治疗组)[MD=-17.19,95%CI(-25.04,-10.54),P<0.00001;MD=-17.19,95%CI(-25.04,-10.54),P<0.00001;MD=-6.76,95%CI(-12.01,-1.50),P<0.05;MD=-11.04,95%CI(-17.14,-4.93),P<0.001;MD=-30.19,95%CI(-41.75,-18.62),P<0.00001;MD=-14.35,95%CI(-20.56,-8.14),P<0.00001]。结论参附注射液联合常规治疗在降低TNF-α、PCT、CRP、IL-1、IL-6及IL-10水平方面优于单用常规治疗,但受纳入的研究质量和数量的限制,仍需多中心、大样本量、高质量RCT验证研究结果。Objective:To systematically review the influence of Shenfu Injection on inflammatory markers in the treatment of septic shock.Method:Randomized controlled trials(RCTs)related to the effect of Shenfu Injection on inflammatory markers in the treatment of septic shock were retrieved from databases of CNKI,Wanfang Data,CBM,VIP,PubMed,Embase and Cochrane Library.RevMan 5.3 software was used for Meta-analysis.Results:A total of 12 RCTs involving 920 patients were included.Meta-analysis showed that the levels of tumor necrosis factor alpha(TNF-α),serum procalcitonin(PCT),C-reactive protein(CRP),interleukin-1(IL-1),IL-6 and IL-10 in the experimental group(Shenfu Injection group)were significantly lower than those of the control group(conventional treatment group)[MD=-17.19,95%CI(-25.04,-10.54),P<0.00001;MD=-17.19,95%CI(-25.04,-10.54),P<0.00001;MD=-6.76,95%CI(-12.01,-1.50),P<0.05;MD=-11.04,95%CI(-17.14,-4.93),P<0.001;MD=-30.19,95%CI(-41.75,-18.62),P<0.00001;MD=-14.35,95%CI(-20.56,-8.14),P<0.00001].Conclusion:Shenfu Injection combined with conventional treatment is superior to conventional treatment alone in reducing the levels of TNF-α,PCT,CRP,IL-1,IL-6 and IL-10.However,due to the quality restriction of the literatures and the small sample size,more rigorous,better quality,larger sample RCTs are needed to further confirm the results.
