蒲公英汤联合芙黄膏文胸治疗急性哺乳期乳腺炎(肝胃郁热证)的临床观察

摘　　要：目的观察蒲公英汤联合芙黄膏文胸治疗急性哺乳期乳腺炎(肝胃郁热证)的临床疗效。方法患者140例按随机数字表法分为治疗组与对照组各70例。两组均予排空乳汁及抗生素治疗,治疗组加予蒲公英汤口服及佩戴芙黄膏文胸外治。观察两组中医证候积分、乳汁通畅时间、局部红肿热痛症状消失时间、超声下治愈时间、抗生素使用时间、炎症指标(血液白细胞计数、C反应蛋白、降钙素原)及不良反应。结果两组治疗前后比较,中医证候总积分及白细胞计数、C反应蛋白、降钙素原等炎症指标均显著下降,且治疗组显著低于对照组(P<0.05)。治疗组乳汁通畅时间、局部红肿热痛症状消失时间、超声下治愈时间及抗生素使用时间均明显短于对照组(P<0.05)。两组患者不良反应差异无统计学意义(P>0.05)。结论蒲公英汤联合芙黄膏文胸治疗急性哺乳期乳腺炎(肝胃郁热证)能够进一步改善患者症状及炎症指标,减少抗生素使用时间,安全有效。Objective:To observe the clinical efficacy of Pugongying Decoction combined with Fuhuang Ointment bra for acute lactation mastitis(syndrome of heat stagnation in liver and stomach).Methods:A total of 140 patients were selected and divided into the treatment group and control group according to random number table method,70 patients in each group.Both groups were treated with emptying breast milk and antibiotics.Pugongying Decoction was taken orally,and Fuhuang Ointment bra was worn in the treatment group.The TCM symptoms integral,the time of lactation recovery,the time of local symptom disappearance,the healing time under ultrasound,the time of using antibiotics,the inflammatory biomarkers(white blood cell count,c-reactive protein and procalcitonin),and the adverse reactions were observed.Results:Compared with before treatment,the total scores of TCM syndromes and inflammatory indicators including white blood cell count,c-reactive protein,and procalcitonin in the two groups after treatment were significantly decreased,and the scores and indicators in the treatment group were significantly lower than those in the control group,the differences were statistically significant(P<0.05);the time of lactation recovery,the time of local symptom disappearance,the healing time under ultrasound and the time of using antibiotics in the treatment group were significantly shorter than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in adverse reactions between the two groups of patients(P>0.05).Conclusion:Pugongying Decoction combined with Fuhuang Ointment bra in the treatment of acute lactation mastitis(syndrome of heat stagnation in liver and stomach)can further improve the clinical symptoms and inflammatory indicators,significantly reduce the time of using antibiotics and accelerate patient recovery,which is safe and effective.
