补益理肺汤联合雾化吸入治疗老年支气管哮喘急性发作的效果及对肺功能的影响

摘　　要：目的探讨补益理肺汤联合雾化吸入治疗老年支气管哮喘(简称哮喘)急性发作的效果及对肺功能的影响。方法选取2014年3月—2016年3月收治的老年哮喘急性发作患者88例作为研究对象,采用随机数字表法分为对照组和观察组,各44例。对照组给予沙丁胺醇+布地奈德雾化吸入治疗,观察组在对照组基础上给予补益理肺汤治疗,以7d为1个用药疗程。用药2个疗程后评估疗效,记录哮鸣音、湿罗音、咳嗽症状消失时间,比较肺功能指标第1秒时间肺活量(FEV1)、第1秒时间肺活量与用力肺活量比值(FEV1/FVC)、呼出一氧化氮(FENO)变化,测定白细胞介素(IL-6、IL-8、IL-33)水平。结果观察组治疗有效率为93.18%(41/44),高于对照组的77.27%(34/44)(P <0.05);观察组哮鸣音、湿罗音、咳嗽消失时间短于对照组(P <0.05);观察组治疗后FEV1、FEV1/FVC高于对照组,FENO低于对照组(P <0.05);观察组治疗后IL-6、IL-8、IL-33水平低于对照组(P <0.05)。结论补益理肺汤联合雾化吸入治疗老年哮喘急性发作可降低气道炎性水平,明显改善肺功能,短时间内缓解临床症状。Objective To investigate the effects of Buyi Lifei Decoction combined with aerosol inhalation in treatment of elderly patients with acute attack of bronchial asthma( BA) and its effect on pulmonary function. Methods Eighty-eight patients with acute attack of BA who were treated between March 2014 and March 2016 were selected as the study subjects and were randomized into the control group and the observation group,44 cases in each group. The control group was treated with salbutamol combined with budesonide aerosol inhalation therapy,while the observation group was additionally treated with Buyi Lifei Decoction. Seven days was 1 course of treatment. After 2 courses of medication,the curative effect was evaluated. The extinction time of wheezing,moist rales and cough symptoms was recorded. The changes of pulmonary function indexes [forced expiratory volume in one second( FEV1),the ratio of forced expiratory volume in one second and forced vital capacity( FEV1/FVC),fractional exhaled nitric oxide( FENO) ] were compared. The levels of IL-6,IL-8 and IL-33 were determined. Results The effective rate in the observation group( 93. 18%,41/44) was higher than that in the control group( 77. 27%,34/44)( P < 0. 05). The extinction time of wheezing,moist rales and cough symptoms in the observation group was shorter than that in the control group( P < 0. 05). After treatment,FEV1 and FEV1/FVC in the observation group were higher than those in the control group,while FENO was lower( P <0. 05). After treatment,the levels of IL-6,IL-8 and IL-33 in the observation group were lower than those in the control group( P < 0. 05). Conclusion The application of Buyi Lifei Decoction combined with aerosol inhalation in treatment of elderly patients with acute attack of BA can reduce the airway inflammatory levels,improve lung function,and relieve clinical symptoms in a short period of time.
