摘　　要：目的:分析与评价我院住院病例中注射用亚胺培南-西司他丁钠的使用情况,为规范临床合理用药提供参考依据。方法:抽取我院2020年1月—6月使用注射用亚胺培南-西司他丁钠的住院病例592例,并对其临床使用情况进行统计与分析。结果:注射用亚胺培南-西司他丁钠使用例数按科室排名,前5位分别是重症医学科、感染科、呼吸内科、血液科、胃肠外科,使用不合理例数共计223例,不合理率为37.67%,不合理使用情况包括用法用量不适宜(42.15%)、遴选药品不适宜(25.56%)、联合用药不适宜(15.70%)、使用疗程过长(12.55%)、未按照规范流程使用(4.04%)。2020年1月—6月,注射用亚胺培南-西司他丁钠各月的药物使用频度(DDDs)分别为3892、4098、4492、4763、5578、6491,各月的药物利用指数(DUI)分别为0.98、1.01、1.03、1.08、1.14、1.29,其DDDs和DUI呈现逐月上升趋势。结论:我院注射用亚胺培南-西司他丁钠临床使用基本合理,但仍有不合理现象存在,在抗菌药物科学化管理背景下,应加强对其临床监管,提高临床合理用药水平。Objective:To analyze and evaluate the use of imipenem cilastatin sodium for injection in our hospital,so as to provide reference for clinical rational drug use.Methods:592 inpatients who used imipenem cilastatin sodium for injection in our hospital from January to June 2020 were selected,and their clinical use was statistically analyzed.Results:The number of cases of imipenem cilastatin sodium for injection was ranked by departments,and the top five were intensive care medicine,infection,respiratory medicine,hematology and gastrointestinal surgery.There were 223 cases of irrational use,with the unreasonable rate of 37.67%.The irrational use of imipenem cilastatin sodium for injection included inappropriate usage and dosage(42.15%),inappropriate selection of drugs(25.56%)and inappropriate combination of drugs(15.70%)The treatment course was too long(12.55%),and it was not used according to the standard process(4.04%).From January to June 2020,the DDDs of imipenem cilastatin sodium for injection were 3892,4098,4492,4763,5578 and 6491,respectively,and the drug utilization index(DUI)of each month were 0.98,1.01,1.03,1.08,1.14 and 1.29,respectively.Conclusion:The clinical use of imipenem cilastatin sodium for injection in our hospital is basically reasonable,but there are still unreasonable phenomena.Under the background of scientific management of antibacterial drugs,we should strengthen the clinical supervision and improve the level of clinical rational drug use.
