芪丹颗粒对2型糖尿病患者胰岛素抵抗及血清CTRP6和CTRP12水平的影响

摘　　要：【目的】研究芪丹颗粒对2型糖尿病脾虚痰瘀证患者胰岛素抵抗、血清CTRP6及CTRP12水平的影响。【方法】将60例2型糖尿病脾虚痰瘀证患者随机分为治疗组和对照组,每组各30例。2组均给予糖尿病基础治疗,在此基础上,治疗组给予芪丹颗粒治疗,对照组给予盐酸二甲双胍片治疗,疗程为8周。观察2组患者治疗前后胰岛素抵抗指数(HOMA-IR)及血清CTRP6和CTRP12水平的变化情况,并评估HOMA-IR与血清CTRP6和CTRP12水平的相关性。【结果】(1)治疗后,2组患者的HOMA-IR均较治疗前明显下降(P<0.05),且治疗组对HOMA-IR的下降作用明显优于对照组(P<0.05)。(2)治疗后,治疗组患者血清CTRP6水平较治疗前明显下降(P<0.05),血清CTRP12水平较治疗前明显升高(P<0.05);而对照组治疗前后血清CTRP6和CTRP12水平均无显著性变化(P>0.05)。组间比较,治疗组对血清CTRP6水平的下降作用和对血清CTRP12水平的升高作用均明显优于对照组(P<0.05)。(3)相关性分析结果提示HOMA-IR与血清CTRP6水平呈正相关(r=0.746,R2=0.557,P<0.01),而与血清CTRP12水平呈负相关(r=-0.802,R2=0.644,P<0.01);血清CTRP6、CTRP12水平是HOMAIR的独立影响因子(P<0.01)。【结论】芪丹颗粒能有效改善2型糖尿病脾虚痰瘀证患者胰岛素抵抗,其作用机制可能与调节血清CTRP6及CTRP12水平有关。Objective To investigate the effect of Qidan Granules on insulin resistance and serum complementC1 q/TNF-related protein 6(CTRP6)and CTRP12 levels in type 2 diabetic patients with the syndrome of spleen deficiency and phlegm-stasis.Methods Sixty type 2 diabetic patients with the syndrome of spleen deficiency and phlegm-stasis were randomly assigned to treatment group and control group,with 30 cases in each group.During 8 weeks of treatment,both groups were given fundamental treatment for diabetes.And additionally,the treatment group was treated with Qidan Granules while the control group was treated with Metformin Hydrochloride Tablets.The changes of insulin resistance estimated with homeostasis model(HOMA-IR),and serum CTRP6 and CTRP12 levels were compared before and after treatment between the two groups.Meanwhile,the correlation of HOMA-IR with serum CTRP6 and CTRP12 levels was also investigated.Results(1)After treatment,HOMA-IR in both groups was significantly lower than that before treatment(P<0.05).The treatment group had stronger effect on decreasing HOMA-IR than the control group(P<0.05).(2)In the treatment group,serum CTRP6 level was decreased and serum CTRP12 level was increased after treatment in comparison with those before treatment(P<0.05).However,no obvious changes of serum CTRP6 and CTRP12 levels were shown in the control group before and after treatment(P>0.05).The intergroup comparison showed that the treatment group had stronger effect on decreasing serum CTRP6 level and on increasing serum CTRP12 level than the control group,the differences being statistically significant(P<0.05).(3)The results of correlation analysis showed that HOMA-IR was positively correlated with the serum CTRP6 level(r=0.746,R2=0.557,P<0.01),and was negatively correlated with serum CTRP12 level(r=-0.802,R2=0.644,P<0.01).In addition,serum CTRP6 and CTRP12 levels were independent influencing factors of HOMA-IR(P<0.01).Conclusion Qidan Granules can effectively improve insulin resistance in type 2 diabetic patients with the
