天麻熄风汤联合米诺地尔酊治疗肝肾不足型斑秃疗效观察

摘　　要：【目的】观察天麻熄风汤联合5%米诺地尔酊治疗肝肾不足型斑秃的临床疗效。【方法】将50例肝肾不足型斑秃患者随机分成试验组30例和对照组20例。试验组给予自拟方天麻熄风汤联合5%米诺地尔酊治疗,对照组给予单纯外用5%米诺地尔酊治疗,疗程为12周。观察2组患者治疗前后的皮损毛发生长评分的变化情况,评价2组患者的中医证候疗效、皮损毛发生长疗效和随访结果。【结果】(1)治疗过程中,试验组和对照组各失访1例,最终48例患者完成全部疗程,其中试验组29例,对照组19例。(2)中医证候疗效:治疗12周后,试验组的治愈率和总有效率分别为89.7%(26/29)和100.0%(29/29),明显优于对照组的42.1%(8/19)和78.9%(15/19),组间比较,差异均有统计学意义(P<0.05)。(3)治疗后,2组患者的皮损毛发生长评分均较治疗前明显降低(P<0.01),且试验组的皮损毛发生长评分明显低于对照组(P<0.05)。(4)皮损毛发生长疗效:试验组的总有效率为96.6%(28/29),对照组为73.7%(14/19),试验组的疗效明显优于对照组,差异有统计学意义(P<0.05)。(5)随访情况:治疗期间,试验组无不良反应情况发生,对照组有1例出现轻微不良反应;治疗结束半年后,试验组无病例复发,对照组有8例出现复发,试验组的复发例数明显少于对照组,差异有统计学意义(P<0.05)。【结论】天麻熄风汤联合5%米诺地尔酊治疗肝肾不足型斑秃疗效确切,既能改善中医临床证候,又能促进皮损毛发生长,且复发率低。Objective To observe the clinical efficacy of Tianma Xifeng Decoction combined with 5%minoxidil tincture for the treatment of alopecia areata with liver and kidney insufficiency sydrome.Methods Fifty alopecia areata patients with liver and kidney insufficiency syndrome were randomly divided into trial group(n=30)and control group(n=20).The trial group was treated with Tianma Xifeng Decoction combined with 5%minoxidil tincture,and the control group was treated with 5%minoxidil tincture alone.The treatment for the two groups lasted 12 weeks.Before and after treatment,the scores of hair growth in the injured skin were observed for the evaluation of the clinical efficacy for traditional Chinese medicine(TCM)syndrome,clinical efficacy for hair growth in the injured skin,and follow-up efficacy in the two groups after treatment.Results(1)During the trial,one case from the trial group and one case from the control group lost to follow-up,and a total of 48 cases in the two groups(29 cases of the trial group and 19 cases of the control group)completed the trial.(2)After treatment for12 weeks,the cure rate and the total effective rate for TCM syndromes in the trial group were 89.7%(26/29)and100.0%(29/29)respectively,being superior to 42.1%(8/19)and 78.9%(15/19)in the control group.The intergroup comparison showed that the differences of the cure rate and the total effective rate for TCM syndromes between the two groups were statistically significant(P<0.05).(3)After treatment,the scores of hair growth in the injured skin of the two groups were obviously decreased in comparison with those before treatment(P<0.01),and the scores in the trial group were much lower than those in the control group(P<0.05).(4)The total effective rate for the hair growth in the injured skin in the trial group was 96.6%(28/29),and that in the control group was73.7%(14/19).The efficacy of the trial group was superior to that of the control group,the difference being statistically significant(P<0.05).(5)During the trial,no adverse reaction occurred i
