隔药灸肠风穴联合马来酸曲美布汀治疗2型糖尿病性腹泻的疗效观察

摘　　要：【目的】观察隔药灸肠风穴联合马来酸曲美布汀治疗2型糖尿病性腹泻的临床疗效。【方法】将70例脾虚湿胜型2型糖尿病性腹泻患者随机分为观察组和对照组,每组各35例。2组患者均给予糖尿病常规治疗,对照组给予马来酸曲美布汀口服,每次0.2 g,每日3次,连续治疗6周。观察组在对照组的基础上给予隔药灸肠风穴治疗,每日1次,每周5次,共治疗6周。治疗6周后,观察2组患者治疗前后自主排便次数、Bristol大便性状分型评分的变化情况,以及中医症状评分、生活质量评分的情况,比较2组患者治疗前后空腹血糖(FBG)、餐后2h血糖(2hPBG)和糖化血红蛋白(HbA1c)水平的变化情况,并评价2组的临床疗效。【结果】(1)研究过程中,观察组失访1例,对照组失访2例。最终观察组34例、对照组33例纳入疗效统计。(2)治疗后,2组患者的自主排便次数和Bristol大便性状分型评分均明显改善(P<0.05),且观察组在改善自主排便次数和Bristol大便性状分型评分方面均明显优于对照组,差异均有统计学意义(P<0.05)。(3)治疗后,2组患者的中医症状评分和生活质量评分均明显改善(P<0.05),且观察组在改善中医症状评分和生活质量评分方面均明显优于对照组,差异有统计学意义(P<0.05)。(4)治疗后,2组患者的FBG、2hPBG和Hb A1c水平均明显改善(P<0.05),但观察组在改善FBG、2hPBG和HbA1c水平方面,与对照组比较,差异无统计学意义(P>0.05)。(5)观察组总有效率为91.18%(31/34),对照组为72.73%(24/33)。观察组疗效优于对照组,差异有统计学意义(P<0.05)。【结论】隔药灸肠风穴联合马来酸曲美布汀治疗脾虚湿胜型2型糖尿病性腹泻,能明显地改善患者的腹泻症状,降低患者的中医症状评分,提高患者的生活质量,有效地调节糖代谢水平,疗效显著。Objective To observe the therapeutic effect of drug partition moxibustion at Changfeng points combined with trimebutine maleate for type 2 diabetic diarrhea.Methods Seventy patients with type 2 diabetic diarrhea of predominant dampness due to spleen deficiency type were randomly divided into observation group and control group,35 cases in each group.The two groups of patients received conventional treatment for diabetes,additionally,the control group received oral use of trimebutine maleate,0.2 g per time,3 times per day,the treatment lasting 6 continuous weeks,and the observation group received drug partition moxibustion at Changfeng points,once a day,5 times a week,the treatment lasting 6 weeks.After 6 weeks of treatment,the changes of spontaneous defecation frequency,Bristol stool type scores,traditional Chinese medicine(TCM)symptoms scores and quality of life scores were observed in the two groups before and after treatment.The levels of fasting blood glucose(FBG),postprandial 2-hour blood glucose(2 hPBG)and glycosylated hemoglobin(HbA1 c)were compared in the two groups before and after treatment,and the clinical efficacy of the two groups was evaluated.Results(1)During the study,one case was lost to follow-up from the observation group and 2 cases from the control group.Finally,34 cases in the observation group and 33 cases in the control group were included in the curative effect statistics.(2)After treatment,the frequency of spontaneous defecation and Bristol stool character scores in the two groups were significantly improved(P<0.05),and the improvement in frequency of spontaneous defecation and Bristol stool characteristics scores of the observation group was significantly superior to that of the control group,the differences being statistically significant(P<0.05).(3)After treatment,the TCM symptoms scores and quality of life scores in the two groups were significantly improved(P<0.05),and the improvement in TCM symptoms scores and quality of life scores of the observation group was significantly superi
