摘  要:目的:探讨SYSMEX XN9100血液分析仪研究参数红细胞-网织通道(RBC-O)及BAYER ADVIA2120血液分析仪研究参数单个细胞血红蛋白(CH)在处理冷凝集标本中的应用价值。方法:收集医院22例患者血常规冷凝集标本(22份),分别采用XN9100网织通道检测法、ADVIA2120检测法、37℃水浴法和血浆置换法4种处理方法进行血常规检测,记录红细胞(RBC)、红细胞平均血红蛋白浓度(MCHC)、平均红细胞体积(MCV)、白细胞(WBC)和血小板(PLT)检测结果。以血浆置换法为参考方法,计算其他方法在RBC、MCHC、MCV、WBC和PLT的5项的相对偏差(%),依据实验室的允许偏差标准RBC(3%)、MCHC(3.5%)、MCV(3.5%)、WBC(7.5%)和PLT(12.5%)确定纠正效果。结果:血浆置换法相比中:(1)XN9100网织通道检测法,用研究参数RBC-O替换RBC并计算MCHC,分别为1例RBC、3例MCHC、1例MCV和1例PLT超过允许偏差,WBC在允许偏差内;(2)ADVIA2120检测法,参数CH通过血红蛋白/单个细胞血红蛋白(HGB/CH)反算得到RBC和MCHC均在允许偏差内,超过允许偏差的分别为1例WBC、1例PLT;(3)37℃水浴法,超过允许偏差范围的分别为7例RBC、4例MCHC、1例MCV、1例PLT,WBC在允许偏差内。结论:CH通过计算可得到消除干扰的RBC值;对相关参数进行判断后可用RBC-O替代RBC。研究参数CH和RBC-O在处理冷凝集标本时均有较好的应用价值。Objective:To explore the application values of the red blood cell-net channel(RBC-O)of research parameter of SYSMEX XN9100 blood analyzer and single cell hemoglobin(CH)of research parameter of BAYER ADVIA2120 blood analyzer in processing samples with cold agglutination.Methods:22 samples with cold agglutination of blood routine examination were collected.And they were examined after they were processed respectively by 4 kinds of handling methods included XN9100 net channel examination method,ADVIA2120 examination method,37℃immersion method and plasmapheresis method.And the examination results of red blood cell(RBC),mean corpuscular hemoglobin concentration(MCHC),mean corpuscular volume(MCV),white blood cell(WBC)and platelet(PLT)were recorded.The plasmapheresis method was used as reference method to calculate the relative deviations(%)of other methods on 5 items,included RBC,MCHC,MCV,WBC and PLT.The allowable deviation standards of laboratory,that included RBC(3%),MCHC(3.5%),MCV(3.5%),WBC(7.5%)and PLT(12.5%),were adopted to confirm the rectification effect.Results:In the comparison between XN9100 net channel examination method,ADVIA2120 examination method,37℃immersion method and plasmapheresis method:1)After the research parameter RBC-0 of XN9100 net channel examination method replaced RBC to calculate MCHC,1 case RBC,3 cases MCHC,1 case MCV and 1 case PLT exceeded allowable deviation,and the result of WBC was within allowable deviation.2)The results of ADVIA2120 indicated that the RBC and MCHC which were inversely computed from the ratio of hemoglobin and cell hemoglobin(HGB/CH)were within allowanced deviation,and the values of WBC(1 case)and PLT(1 case)exceeded to allowable deviation.3)The results of 37℃immersion method indicated that the values of RBC(7 cases),MCHC(4 cases),MCV(1 case)and PLT(1 case)exceeded the range of allowable deviation,and that of WBC was within allowable deviation.Conclusion:CH can obtain the RBC value without interference by calculation,and RBC-0 can replace RBC after the relevant
