摘 要:目的观察初次诊断为2型糖尿病患者血清中胎球蛋白-B(Fetuin-B)、前列环素(prostacyclin,PGI2)水平的变化及与胰岛素抵抗(Insulin resistance,IR)的相关性。方法选取2019年10月至2020年4月在牡丹江心血管病医院初次诊断2型糖尿病(Type 2 Diabetes Mellitus,T2DM)患者60例,按照体质量指数(BMI)分为超重或肥胖T2DM组30例(BMI≥24 kg/m2),非肥胖T2DM组30例(BMI<24 kg/m2),另选取30例健康者为对照组。测定血清中Fetuin-B、PGI2的代谢产物6-keto-PGF1α含量及基因表达情况。结果超重或肥胖组和非肥胖组两组与对照组比较糖化血红蛋白(HbA1c)、胰岛素抵抗指数(HOMA-IR)差异有统计学意义(P<0.05);超重或肥胖组和非肥胖组血清Fetuin-B含量和mRNA表达水平高于对照组(P<0.05),且超重或肥胖组比非肥胖组升高更明显(P<0.05);超重或肥胖组和非肥胖组血清6-keto-PGF1α含量和mRNA表达水平低于对照组(P<0.05),且超重或肥胖组比非肥胖组降低更显著(P<0.05);血清Fetuin-B与HOMA-IR呈正相关(P<0.05),6-keto-PGF1α与HOMA-IR呈负相关(P<0.05)。结论初次诊断T2DM患者存在IR,且血清中Fetuin-B、PGI2的代谢产物6-keto-PGF1α水平与IR具有相关性。Objective To observe the changes of serum Fetuin-b(fetuin-b)and prostacyclin(PGI2)levels in newly diagnosed patients with type 2 diabetes mellitus(T2DM)and their correlation with insulin resistance(IR).Methods Totally 60 patients with Type 2 Diabetes Mellitus(T2DM)who were initially diagnosed in Mudanjiang Cardiovascular Hospital from October 2019 to April 2020 were disintegrated into the overweight or obese T2DM group(BMI≥24 kg/m2)and non-obese T2DM group(BMI<24 kg/m2)according to body mass index(BMI).Another 30 healthy patients were selected as the control group.The content and gene expression of Fetuin-B and PGI2 metabolite 6-keto-PGF1 in serum were determined.Results There were statistically significant differences in HbA1c and insulin resistance index(HOMA-IR)between the overweight or obese group and the non-obese group and the control group(P<0.05).The serum Fetuin-B content and mRNA expression level in the overweight or obese group and the non-obese group were higher than those in the control group(P<0.05),and the increase in the overweight or obese group was more significant than that in the non-obese group(P<0.05).The levels of serum 6-keto-PGF1 and mRNA in the overweight or obese group and the non-obese group were lower than those in the control group(P<0.05),and the decrease in the overweight or obese group was more significant than that in the non-obese group(P<0.05).Serum Fetuin-b was positively correlated with HOMa-IR(P<0.05),and 6-keto-pGF1 was negatively correlated with homa-IR(P<0.05).Conclusion IR exists in newly diagnosed T2DM patients,and the level of serum Fetuin-B and PGI2 metabolite 6-keto-PGF1αis correlated with IR.
