摘　　要：目的:建设以慢病管理系统为核心,辅助以肾病实验室、数字化病理、随访等多个功能模块的综合信息系统。该系统服务于肾内慢病全治疗周期,为患者提供从线上到医院的全流程信息化服务体系。方法:以肾内CKD临床治疗的实际痛点为出发点,搭建了联通互联网以及医院内部信息系统为一体的综合性CKD管理平台,医患能够在系统上获取所需要的数据信息,智能化算法降低了医护的工作压力,提升了患者的就医体验。结果:该系统为CKD患者以及肾内科医生护士提供涵盖了全治疗周期的信息化支持系统,能够切实帮助医护提升工作效率和操作安全,同时降低患者的负担并改善生活质量。结论:通过本系统的实践,摸索了未来CKD慢病管理系统发展的方向,值得借鉴。Objective:To build a comprehensive information system with a chronic disease management system as its core and assisted by multiple functional modules such as a nephrology laboratory,digital pathology,and follow-up.This system serves the entire treatment cycle of chronic kidney disease and provides patients with a full-process information service system from online to the hospital.Methods:Taking the actual pain point of clinical treatment of CKD in the kidney as the starting point,a comprehensive CKD management platform is established,which integrates the Unicom Internet and the hospital’s internal information system.The doctors and patients can obtain the required data information on the system,and the intelligent algorithm reduces the working pressure of medical care has improved the patient’s medical experience.Results:The system provides CKD patients and nurses of nephrologists with an information-based support system covering the entire treatment cycle,which can effectively help doctors improve work efficiency,improve operation safety,and at the same time reduce the burden on patients and improve the quality of life.Conclusion:Through the practice of this system,it explores the development direction of CKD chronic disease management system in the future,which is worth learning from.
