摘 要:目的探讨使用电针结合不同频率的重复经颅磁刺激方法治疗中风后患有运动性失语患者的治疗效果。方法采用随机数字表法,将符合标准的中风后失语症患者随机分为高频组、低频组、对照组。在常规治疗的基础上,高频组患者进行左侧半球Broca区高频刺激(5 Hz)+电针+语言康复治疗;低频组患者进行右侧半球Broca对应区低频刺激(1 Hz)+电针+语言康复治疗;对照组进行电针+语言康复治疗。各组治疗均每日1次,每周5次,共20次,总疗程共4周。治疗前后采用日常生活活动能力(ADL)量表、汉密尔顿抑郁量表(HAMD)进行评分以对比3组治疗效果。结果高频组、低频组总有效率(80.00%)显著高于对照组(63.63%)(P<0.05);高频组、低频组ADL评分与HAMD评分分别与对照组相比差异有统计学意义(P<0.05),高频组与低频组组间相比差异无统计学意义(P>0.05)。结论电针结合高频或低频重复经颅磁刺激治疗中风后运动性失语具有良好的疗效,两种治疗方法均能提高中风后失语的言语功能状态,改善患者的临床症状。Objective To research the clinical result of electroacupuncture combined with repetitive transcranial magnetic stimulation at different frequencies in the treatment of aphasia after stroke.Methods The patients with aphasia after stroke were randomly divided into high frequency group,low frequency group and control group.On the basis of conventional treatment,patients in the high-frequency group received high-frequency stimulation(5 Hz)+electroacupuncture+speech rehabilitation in the Broca area of the left hemisphere.The low-frequency group received lowfrequency stimulation(1 Hz)+electroacupuncture+speech rehabilitation in the corresponding region of Broca in the right hemisphere.The control group received electroacupuncture plus speech rehabilitation.Each group was treated once a day,5 times a week,a total of 20 times,a total of 4 weeks.Activity of daily life(ADL)scale and Hamilton depression scale(HAMD)were used to compare the treatment effect of the three groups before and after treatment.Results The total effective rate(80.00%)of the high frequency group and the low frequency group was obviously higher than that of the control group(63.63%)(P<0.05).The ADL score and HAMD score of the high frequency group and the low frequency group were statistically different from the control group(P<0.05),while there was no significant difference between the high frequency group and the low frequency group(P<0.05).Conclusion Electroacupuncture combined with high-frequency repetitive transcranial magnetic stimulation or low-frequency repetitive transcranial magnetic stimulation has a good effect in the treatment of motor aphasia after stroke.Both methods can improve the speech function state of aphasia after stroke and improve the clinical symptoms of patients.
