摘 要:目的探讨2型糖尿病(T2DM)患者脑白质病变(WML)与信息处理速度的相关性分析。方法该研究纳入2019年7月—2020年1月T2DM患者62例和无糖尿病对照组24例,进行头颅磁共振(MRI)检查,采用Fazekas量表评估WML严重程度,并采用连线试验A(TMT-A)进行信息处理速度检测,对比两组的一般临床资料、Fazekas评分、连线试验A完成时间,分析T2DM患者Fazekas评分与糖化血红蛋白(HbA1c)、TMT-A完成时间的相关性,并采用多元线性回归分析T2DM患者TMT-A完成时间的影响因素。结果T2DM患者的空腹血糖(FBG)、HbA1c水平、Fazekas评分、TMT-A完成时间均高于对照组,差异有统计学意义(P<0.05)。T2DM患者的Fazekas PVH、Fazekas总分与HbA1c存在正相关,Fazekas各项评分均与TMT-A完成时间存在相关性。在多元线性回归分析中,Fazekas PVH为T2DM患者TMT-A完成时间的独立影响因素。结论与无糖尿病者相比,T2DM患者的WML程度较重,且信息处理速度较慢,而脑室旁WML可能是影响T2DM患者信息处理速度的独立影响因素。Objective To explore the correlation between white matter lesions(WML)and mental processing speed in patients with type 2 diabetes(T2DM).Methods This study included from July 2019 to January 2020 T2DM patients with 62 cases and control group 24 cases without diabetes,cranial magnetic resonance(MRI)examination,adopting Fazekas WML severity scale evaluation,and the connection test A(TMT-A)was used to detect the information processing speed,the general clinical data and compared two groups Fazekas score,the attachment test A complete time,analysis of T2DM patients Fazekas scores and glycosylated hemoglobin(HbA1c),the correlation of TMT-A completion time,multiple regression analysis was used to analyze the influencing factors of TMT-A completion time in T2DM patients.Results The fasting blood glucose(FBG),HbA1c,Fazekas bescore,and the complete time of TMT-A of T2DM patients were higher than those of the control group,the difference was statistically significant(P<0.05).Fazekas PVH,Fazekas total scores of T2DM patients were positively correlated with Hb A1 c,while each part of Fazekas scores was correlated with TMT-A.In multiple linear regression analysis,Fazekas PVH was an independent influencing factor of TMT-A in patients with T2DM.Conclusion Compared with not-diabetic controls,T2DM patients have more severe WML and slower mental processing speed.Paraventricular WML may be an independent factor affecting the mental processing speed of T2DM patients.
