摘 要:目的分析七氟醚吸入麻醉在体外循环转流期间对肌钙蛋白Ⅰ的影响。方法该院在2013年2月—2019年8月期间共计进行瓣膜置换术手术50例,将其作为观察对象,按照入院的先后顺序对患者进行如下分组,观察组25例患者采用七氟醚吸入麻醉,对照组25例患者采用丙泊酚麻醉,对比两组患者的麻醉效果、不同时段肌钙蛋白Ⅰ水平及不良反应。结果观察组患者术后HR为(70.3±3.2)次/min,SBP为(127.4±7.3)mmHg,DBP为(84.9±5.2)mm Hg,BIS指数为(82.4±3.7),各项数据较之对照组差异有统计学意义(t=7.076、5.215、7.207、7.137,P<0.05);观察组患者T0、T1、T2时段肌钙蛋白Ⅰ水平分别是(0.43±0.12)μg/L、(0.45±0.17)μg/L、(0.48±0.21)μg/L,数据较之于对照组,差异无统计学意义(t=0.500、0.220、0.329,P>0.05),但T3、T4时段肌钙蛋白Ⅰ水平为(0.49±0.15)μg/L、(0.55±0.14)μg/L,数据高于对照组,差异有统计学意义(t=3.876、7.582,P<0.05);两组患者不良反应数据,观察组为12%,对照组为8%,差异无统计学意义(χ^2=0.222,P>0.05)。结论七氟醚吸入麻醉在体外循环转流期间可以抑制肌钙蛋白Ⅰ,降低其浓度,而且对患者血流动力学影响较小,不良反应风险低,故而推荐使用。Objective To analyze sevoflurane inhalation anesthesia during extracorporeal circulation turn flow affectⅠtroponin.Methods The hospital in February 2013 to August 2019,a total of 50 cases of valve replacement surgery,as the observation object,according to the order of admission to the following group of patients,observation group of 25 patients with sevoflurane inhalation anesthesia,control group of 25 patients with propofol anesthesia,compared the anesthetic effect of two groups of patients,different timesⅠtroponin levels and adverse reactions.Results The postoperative HR of the observation group was(70.3±3.2)times/min,the SBP was(127.4±7.3)mmHg,the DBP was(84.9±5.2)mmHg,and the BIS index was(82.4±3.7).Compared with the control group,the difference was statistically significant(t=7.076,5.215,7.207,7.137,P<0.05);the levels of troponin I during T0,T1,and T2 in the observation group were(0.43±0.12)μg/L,(0.45±0.17)μg/L,(0.48±0.21)μg/L.Compared with the control group,the difference was not statistically significant(t=0.500,0.220,0.329,P>0.05),but the level of troponin I during T3 and T4 is(0.49±0.15)μg/L,(0.55±0.14)μg/L,the data is higher than the control group,the difference was statistically significant(t=3.876,7.582,P<0.05);the adverse reaction data of the two groups of patients,the observation group is 12%,the control group is 8%,the difference was not statistically significant(χ2=0.222,P>0.05).Conclusion Sevoflurane inhalation anesthesia during extracorporeal circulation turn flow can inhibit the troponinⅠ,reduce its concentration,and the influence on hemodynamics in patients with small,low risk of adverse reactions,so it is recommended to use.
