探究宫腔镜手术治疗剖宫产术后子宫瘢痕妊娠临床效果

摘　　要：目的分析宫腔镜手术治疗剖宫术后子宫瘢痕妊娠(CSP)效果。方法纳入该院2016年3月—2018年3月76例CSP患者,回顾性分析其资料;根据术式不同分为传统清宫组(传统清宫术)、栓塞组(子宫动脉栓塞术)、宫腔镜组(宫腔镜手术)。记录手术指标(手术时间,术中出血量)、7 d血HCG下降程度、HCG复常耗时、术后阴道流血时间、月经恢复时间、住院时间、并发症发生率。结果与传统清宫组比,宫腔镜组手术时间短、术中出血量少,7 d血HCG下降程度高,HCG复常耗时、住院时间短,并发症发生率低,差异有统计学意义(P<0.05)。宫腔镜组上述指标与栓塞组比较,差异无统计学意义(P>0.05)。结论宫腔镜手术用于CSP中能降低风险,促进康复,优于传统清宫术。Objective To analyze the effect of hysteroscopy in the treatment of cicatricial pregnancy(CSP)after cesarean section.Methods 76 cases of CSP from March 2016 to March 2018 in the hospital were included and analyzed retrospectively.According to the different operation methods,they were divided into traditional uterine cleaning group(traditional uterine cleaning operation),embolization group(uterine artery embolization),hysteroscopy group(hysteroscopy operation).The index of operation(the operation time,the amount of bleeding during operation),the degree of HCG decrease in 17 days,the time of HCG recovery,the time of vaginal bleeding,the time of menstrual recovery,the time of hospitalization and the incidence of complications were recorded.Results Compared with the traditional uterine cleaning group,the operation time of hysteroscopy group was shorter,the amount of bleeding during operation was less,the decreased level of HCG in blood was higher in 7 days,the time for HCG to return to normal,the time for hospitalization was shorter,and the incidence of complications was lower,the difference was statistically significant(P<0.05).The above indexes in hysteroscopy group were compared with those in embolization group,the difference was not statistically significant(P>0.05).Conclusion Hysteroscopy can reduce the risk and promote the recovery of CSP,which is better than traditional uterine cleaning.
