摘　　要：目的:分析检测脓毒症休克患者血清N末端脑钠肽前体(NT-proBNP)水平对发生左心室舒张功能障碍(LVDD)的预测价值。方法:选取2017年12月-2019年10月入院24 h内转变为脓毒症休克患者72例,根据是否发生LVDD分为A组(已发生)39例和B组(未发生)33例。收集两组患者临床资料,并分析发生LVDD的独立危险因素及NT-proBNP水平对发生LVDD的预测价值。结果:两组NT-proBNP、血肌酐及心肌肌酐蛋白Ⅰ(CTnⅠ)水平比较,差异有统计学意义(P<0.05)。将上述存在差异的因素应用Logistic回归分析显示,NT-proBNP是预测脓毒症休克患者发生LVDD的独立危险因素(P<0.05)。结论:检测脓毒症休克患者的NT-proBNP水平对其是否并发LVDD有显著预测价值,可为临床相关治疗提供有效借鉴。Objective:To analyze the predictive value of serum N-terminal pro-B-type natriuretic peptide(NT-proBNP)for left ventricular diastolic dysfunction(LVDD)in septic shock patients.Methods:From December 2017 to October 2019,72 patients who were developed septic shock within 24 h after admitted to hospital were selected.According to whether LVDD had occurred,they were divided into two groups with 39 patients in group A(already occurred)and 33 patients in group B(not occurrence).Clinical data of the two groups of patients were collected,and the independent risk factors for LVDD and the predictive value of NT-proBNP level for LVDD were analyzed.Results:The levels of NT-proBNP,serum creatinine and cardiac troponinⅠ(CTnⅠ)in the two groups were compared,with the differences were statistically significant(P<0.05).Logistic regression analysis by the above difference factors showed that NT-probNP was an independent risk factor for predicting LVDD in septic shock patients(P<0.05).Conclusion:The detection of NT-proBNP level in septic shock patients has significant predictive value for their whether concurrent LVDD,which can provide effective reference for the development of clinical related treatment.
