摘　　要：目的探讨应用内置外固定架(internal fixation,INFIX)治疗骨盆前环损伤的临床疗效。方法回顾性分析采用INFIX系统治疗骨盆前环损伤的21例患者的临床资料,其中骨盆单侧单纯前环损伤4例,双侧耻骨支、坐骨支损伤15例,耻骨联合分离联合耻骨支、坐骨支损伤1例,单纯耻骨联合分离1例。采用Tile分型记录骨盆损伤类型,观察骨折愈合情况并记录手术并发症,术后骨折复位评估采用Matta评价标准,术后骨盆功能评估采用Majeed评价标准。结果21例骨盆前环损伤患者中只固定前环8例,同时固定前环和后环13例;术后骨折复位优15例,良6例,优良率100%;术后骨盆功能优14例,良7例;1例发生股神经损伤,1例出现骨折不愈合。患者去除内置固定架平均时间为5个月(10周~11个月)。结论应用INFIX系统治疗骨盆前环损伤疗效确切,是一种安全有效的临床微创治疗方法。Objective To investigate the clinical effect of internal fixation(INFIX)in the treatment of anterior pelvic ring injury.Methods The clinical data of 21 patients with pelvic anterior ring injury treated by INFIX system were were retrospectively analyzed.There were 4 cases of unilateral anterior ring injury,15 cases of bilateral pubic and sciatic branches injury,1 case of pubic symphysis separation,1 case of pubic symphysis separation,and 1 case of simple pubic symphysis separation.The injury types of pelvis were recorded by Tile classification,and the healing of fracture and the complications of operation were observed.The reduction standard of postoperative fracture was evaluated by Matta score,and the postoperative pelvic function was evaluated by Majeed score.Results In 21 patients with anterior pelvic ring injury,only 8 cases were fixed with anterior ring,and 13 cases were fixed with anterior ring and posterior ring at the same time.15 cases were excellent in postoperative fracture reduction and 6 cases were good.The excellent and good rate was 100%.Postoperative pelvic function was excellent in 14 cases and good in 7 cases.Femoral nerve injury was in 1 case,and fracture nonunion in 1 case.The average time of removal of anterior internal fixation was 5 months(from 10 weeks to 11 months).Conclusion Infix system is a safe,effective and minimally invasive method in the treatment of anterior pelvic ring injury.
