摘  要:目的深入了解2017-2018年广州市流行的乙型流感病毒的基因特征,明确乙型流感病毒的变异和流行特点,与WHO推荐的疫苗株进行比对分析匹配度,为流感监测防控提供科学依据。方法将2017年12月至2018年4月从广州市流感监测哨点医院采集的乙型流感病例的咽拭子标本进行分离培养、核酸提取、逆转录-聚合酶链反应和序列测定,通过构建血凝素(HA)和神经氨酸酶(NA)分子进化树进行分析,并与疫苗株进行比较,分析临床株的变异情况。结果2017-2018年广州市Yamagata系和Victoria系的乙型流感病毒同时存在,Yamagata系活动水平较Victoria系高。Yamagata系所测病毒株与WHO推荐的2017-2018年乙型流感疫苗株B/Phuket/3073/2013有更近的亲缘性,且毒株的HA蛋白均携带L187Q和M266V的氨基酸突变,变异未涉及抗原决定簇,个别毒株存在其他位点发生突变的情况;Victoria系所测病毒株与WHO推荐的2017-2018年流感疫苗株B/Brisbane/60/2008亲缘性更近,且毒株的HA蛋白均携带I132V、N144D和A214T的氨基酸突变,变异涉及2个抗原决定簇,个别毒株存在其他位点发生突变的情况。同时,检测到1株BY-HA/BV-NA系间重配病毒。结论2017-2018年度的三价流感疫苗株未能对广州市人群形成良好的保护作用。Objective To investigate the genetic characteristics of influenza B virus circulating in Guangzhou from 2017 to 2018,clarify the mutation and epidemic characteristics of influenza B virus,provide scientific basis for influenza surveillance and prevention by making comparison with the vaccine strains recommended by World Health Organization(W H O)and analyzing the matching degree.Methods The throat swab specimens of influenza B cases collected from the sentinel hospital for influenza surveillance in Guangzhou were subjected to isolation,nucleic acid extraction,reverse transcription-polymerase chain reaction and sequence determination.Hemagglutinin(H A)and neuraminidase(N A)molecular evolutionary trees were constructed for analysis,and the variation of clinical strains was compared with vaccine strains.Results The influenza B viruses of Yamagata lineage and Victoria lineage were both found to be existent in Guangzhou from 2017 to 2018,and the activity of B Yamagata lineage virus was higher than that of B Victoria lineage virus.The tested virus strains of Yamagata lineage were more closely related to influenza vaccine strain B/P h u ket/3073/2013 recommended by WHO from 2017 to 2018,and the HA proteins of the strains carried amino acid mutations of L187Q and M266V,which did not involve antigenic determinants.Individual strains had mutation at other sites.The tested virus strains of Victoria lineage were more closely related to influenza vaccine strain B/Brisbane/60/2008 recommended by W H O from 2017 to 2018,and the HA proteins of the strains carried amino acid mutations of I132V,N144D and A214T,which involved two antigenic determinants.Individual strains had mutation at other sites.At the same time,a strain of B Y-H A/B V-N A inter-line reassortant was found.Conclusions The trivalent influenza vaccine strains from 2017 to 2018 fail to provide a good protection to the population in Guangzhou.
