摘 要:目的系统评价脂蛋白α[Lipoproteinα,Lp(α)]对冠状动脉易损斑块的预测价值.方法以Lp(α)、易损斑块、不稳定斑块、冠状动脉、冠心病、Lipoproteinα、Lipoprotein(α)、vulnerable plaque、unstable plaque、coronary vessel、coronary artery、coronary heart disease、coronary disease等为检索词,检索PubMed、Embase、Cochrane Library、CNKI、CBM以及万方平台从建库到2020年7月的关于Lp(α)与冠状动脉易损斑块相关性的临床研究.采用NOS质量评分量表对文献进行质量评价,应用RevMan 5.3软件及Stata 12.0软件对纳入的文献进行Meta分析.结果经筛选后最终纳入文献11篇,1351例患者,易损斑块组717例,稳定斑块组634例.Meta分析结果显示,易损斑块组中患者血清Lp(α)水平较稳定斑块组明显升高(SMD=1.27,95%CI 0.77~1.77).结论人血清中Lp(α)可能是冠状动脉易损斑块形成的重要危险因素,其含量的升高可能具有预测急性心血管事件发生的意义.Objective To systematically evaluate the predictive value of lipoprotein α[Lp(α)]in coronary artery vulnerable plaques.Methods PubMed,Embase,Cochrane Library,CNKI,CBM and Wanfang Platform were indexed by Lp(α),vulnerable plaques,unstable plaques,coronary arteries,coronary artery disease to search all the clinical researches from the above data bases on the correlation between Lp(α)and coronary vulnerable plaques since the establishment date of the Library to June 2020.NOS quality scale was used to evaluate the quality of the literature and RevMan 5.3 and Stata 12.0 were used for Meta analysis of the included literature.Results After screening,11 references were included,including 1351 patients,717 in the vulnerable plaque group and 634 in the stable plaque group.The Meta analysis results showed that the serum Lp(α)level of patients in the vulnerable plaque group was significantly higher than that in the stable plaque group(SMD—1.27,95%CI 0.77-1.77).Conclusion Human serum Lp(α)may be an important risk factor for the formation of coronary artery vulnerable plaques and its increase may have the significance to predict the occurrence of acute cardiovascular events.
