产前超声三血管观诊断肺动脉吊带

摘　　要：目的探讨产前胎儿超声三血管观诊断肺动脉吊带(PAS)的应用价值。方法收集分析2006年6月至2018年6月在我院进行产前超声筛查诊断的4例肺动脉吊带异常胎儿,总结其在三血管观(三血管肺动脉分叉切面、三血管切面及三血管气管切面)的超声图像特征,并与胎儿产后超声心动图结果进行对比分析。结果4例肺动脉吊带均有典型声像图表现:三血管观未见左肺动脉从主肺动脉发出,显示其发自右肺动脉后向左侧走形,包绕气管形成血管环。4例异常胎儿中:2例产前诊断为单纯的左肺动脉吊带;1例产前诊断左肺动脉吊带合并室间隔膜周部缺损;1例产前诊断左肺动脉吊带合并唇腭裂畸形;3例皆选择剖宫产,产后超声心动图证实了产前的诊断;1例异常胎儿选择宫内引产,产后家属强烈拒绝病理解剖检查。结论胎儿肺动脉吊带畸形产前超声检查具有特征性的表现,三血管观能够诊断肺动脉吊带。Objective To explore the value of prenatal fetal ultrasound trivascular view in the diagnosis of pulmonary artery sling(PAS).Methods Four fetuses with abnormal pulmonary artery sling diagnosed by prenatal ultrasound screening in our hospital from June 2006 to June 2018 were collected and analyzed.The ultrasonographic features of tri-vessel view(three-vessel pulmonary artery bifurcation section,three-vessel section and three-vessel trachea section)were summarized and compared with the results of fetal postpartum echocardiography.Results The typical sonographic findings of pulmonary artery sling in 4 cases were as follows:the three vessels did not show that the left pulmonary artery originated from the main pulmonary artery,showing that the left pulmonary artery came from the right pulmonary artery and formed a vascular ring around the trachea.Among the 4 cases with abnormal fetus,2 cases were diagnosed as simple left pulmonary artery sling,1 case was diagnosed as left pulmonary artery sling with interventricular diaphragm defect,1 case was diagnosed as left pulmonary artery sling complicated with cleft lip and palate,and 1 case was diagnosed as left pulmonary artery sling with cleft lip and palate.Cesarean section was selected in the first 3 cases,and prenatal diagnosis was confirmed by postpartum echocardiography.In the last case with abnormal fetus,intrauterine induction of labor was selected,and the postpartum family strongly refused pathological anatomical examination.Conclusion Prenatal ultrasonography of fetal pulmonary artery sling malformation has characteristic manifestations.Trivascular view can be used to diagnose pulmonary artery sling.
