磁共振结合磁共振胰胆管成像在胆管梗阻疾病诊断中的应用价值

摘　　要：目的探讨磁共振成像(MRI)结合磁共振胰胆管成像(MRCP)及原始图像在胆管梗阻疾病中的临床价值。方法回顾性分析我院2016年1月份到2019年3月份126名经手术病理证实胆管梗阻疾病患者的MRI结合MRCP及原始图像以及MIP重建图像的诊断结果和病理学检查结果。结果MRCP、MRI结合MRCP及原始图像对胆管梗阻疾病患者定性诊断准确率分别为84.9%(107/126)、96.8%(122/126);均较清晰地显示了梗阻性疾病的梗阻部位和形态。MRI结合MRCP检验结果和临床病理学检验结果比较,均具有较高的临床检验率,差异无统计学意义(P>0.05)。结论对胆管梗阻疾病患者行MRI结合MRCP联合扫描及MRCP原始图像的分析,能够提高胆管梗阻性疾病的准确率,临床上应进一步推广应用。Objective To investigate the clinical value of MRI combined with MRCP and original images in bile duct obstruction.Methods MRI combined with MRCP and original images and MIP reconstructed images of the diagnostic results and pathological examination results of 126 patients with bile duct obstruction disease confirmed by surgical pathology in our hospital from January 2016 to March 2019 were retrospectively analyzed.Results The accuracy of MRCP in qualitative diagnosis of patients with bile duct obstruction was 84.9%(107/126),and the accuracy of MRI combined with MRCP and original images in qualitative diagnosis of patients with bile duct obstruction was 96.8%(122/126);The obstruction site and morphology of obstructive disease were clearly demonstrated.The results of MRI combined with MRCP were compared with those of clinicopathology,showing a high clinical test rate,with no statistical significance(P>0.05).Conclusion By analyzing MRI combined with MRCP and the original MRCP images of patients with bile duct obstruction,the accuracy of bile duct obstruction can be improved,which should be further popularized in clinical practice.
