摘 要:目的探讨64排螺旋CT血管成像(64-SCTA)诊断颅内动脉瘤影像学特点及其价值。方法选择2017年1月~2019年12月南昌大学第四附属医院接诊的经数字减影血管造影(DSA)确诊的60例颅内动脉瘤患者(63个动脉瘤)作为研究对象,均接受64-SCTA检查。以DSA检查结果为金标准,分析64-SCTA诊断颅内动脉瘤价值。结果64-SCTA诊断颅内动脉瘤检出率、瘤体短轴、瘤体长轴、瘤颈宽度与瘤体定位质量评分与DSA比较,差异无统计学意义(P>0.05);64-SCTA诊断颅内动脉瘤三维关系、瘤体形态质量评分高于DSA[(2.34±0.21)分vs.(2.01±0.12)分、(2.23±0.31)分vs.(1.96±0.17)分],差异有统计学意义(P<0.05)。64-SCTA平扫病灶为高密度影且呈类圆形,增强扫描病灶显著强化,均匀高密度影,对比剂填充;容积再现(VR)重建动脉瘤瘤体与周围结构关系、三维关系和瘤体形态显示清晰;最大密度投影(MIP)可准确定位动脉瘤,但图像清晰度较VR欠佳;多平面重建(MPR)重建可准确定位动脉瘤,瘤体光滑度优良且形态清晰,三维空间关系显示欠佳。结论64-SCTA诊断颅内动脉瘤准确性较高,配合强大后处理技术,能准确定位瘤体,清晰显示瘤体大小、形态与空间关系,为临床手术方案的制订提供参考依据。Objective To investigate the imaging characteristics and value of 64-slice spiral CT angiography(64-SCTA)in the diagnosis of intracranial aneurysms.Methods A total of 60 patients with intracranial aneurysms(63 aneurysms)confirmed by digital subtraction angiography(DSA)admitted to the Fourth Affiliated Hospital of Nanchang University from January 2017 to December 2019 were selected as the research subjects,all of whom underwent 64-SCTA examination.Using DSA results as the gold standard,the value of 64-SCTA in the diagnosis of intracranial aneurysms was analyzed.Results The detection rate,the short axis of the tumor,the long axis of the tumor,the width of the tumor neck,and the tumor location quality score of 64-SCTA for the diagnosis of intracranial aneurysms were compared with DSA,there were no statistically significant differences(P>0.05).The three-dimensional relationship and tumor morphology quality scores of 64-SCTA for the diagnosis of intracranial aneurysm were higher than those of DSA([2.34±0.21]points vs.[2.01±0.12]points,[2.23±0.31]points vs.[1.96±0.17]points),and the differences were statistically significant(P<0.05).64-SCTA plain scan lesions were high-density shadows and round-like,enhanced scan lesions were significantly enhanced,uniform high-density shadows,contrast-filled.The volume rendering(VR)reconstructed the relationship between the aneurysm body and the surrounding structure,three-dimensional relationships and tumor morphology were clearly displayed.The maximum intensity projection(MIP)could accurately locate aneurysms,but the image clarity was poorer than VR.The multi-planar reconstruction(MPR)reconstruction could accurately locate aneurysms with excellent smoothness of the tumor and the shape was clear,but the three-dimensional spatial relationship was not well displayed.Conclusion 64-SCTA has high accuracy in diagnosing intracranial aneurysms.Combined with powerful post-processing technology,it can accurately locate tumors,clearly show tumor size,morphology and spatial relationship,and
