不同剂量瑞芬太尼复合丙泊酚对无痛胃肠镜检查患者的清醒镇痛效果

摘　　要：目的分析不同剂量瑞芬太尼复合丙泊酚在无痛胃肠镜患者检查中的应用价值。方法选取2017年7月-2019年7月北京市仁和医院收治的胃肠镜检查患者120例为研究对象,均使用瑞芬太尼复合丙泊酚,根据瑞芬太尼使用剂量不同分为A、B组,每组60例。A组给予瑞芬太尼0.5μg/kg,B组给予瑞芬太尼1.0μg/kg,比较2组检查前后生理指标、胃肠镜检查期间不良反应发生率及清醒后镇痛效果。结果2组检查后生理指标(收缩压、舒张压、心率、呼吸)比较差异均无统计学意义(P>0.05);A组无痛胃肠镜检查期间不良反应(体动、低血压、恶心呕吐、头晕)发生率均低于B组(P<0.05);2组清醒后疼痛数字评价量表(NRS)评分比较差异无统计学意义(P>0.05)。结论无痛胃肠镜检查中应用瑞芬太尼0.5μg/kg+丙泊酚1 mg/kg,可兼顾其检查安全性与术后镇痛效果,值得临床推广应用。Objective Objective to analyze the application value of different doses of remifentanil combined with propofol in painless gastrointestinal endoscopy.Methods 120 patients undergoing gastrointestinal endoscopy admitted to Beijing Renhe Hospital from July 2017 to July 2019 were selected as the study subjects.They all used remifentanil combined with propofol,and were classified into A,Group B,60 cases in each group.Group A was given remifentanil 0.5μg/kg for analgesia,group B was given remifentanil 1.0μg/kg.The physiological indicators before and after the examination,the incidence of adverse reactions during gastrointestinal endoscopy,and the analgesic effect after wakefulness were compared between the two groups.Results There was no significant difference in physiological indexes(systolic blood pressure,diastolic blood pressure,heart rate,respiration)between the two groups after examination(P>0.05).The incidence of adverse reactions during painless gastrointestinal endoscopy in group A was lower than that in group B(P<0.05).There was no significant difference in the scores of the two groups on the Numerical Evaluation Scale(NRS)of Pain after Awakening(P>0.05).Conclusion The application of remifentanil(0.5μg/kg)+propofol(1 mg/kg)in painless gastrointestinal endoscopy can take into account the safety of examination and postoperative analgesic effect.
