摘 要:目的:探讨紫杉烷类抗肿瘤药物相关心血管毒性。方法:检索国家药品不良反应监测系统及CNKI,Wanfang,PubMed,Emabse数据库,收集紫杉烷类抗肿瘤药物相关心血管毒性,总结和分析心血管毒性类型、发生时间、转归及影响因素。结果:共纳入75例患者,紫杉醇、多西他赛、白蛋白结合型紫杉醇、紫杉醇脂质体相关心血管毒性分别为43,26,5,1例;报道的紫杉醇最常见的心血管毒性类型心律失常、高血压、冠状动脉疾病;多西他赛最常见的心血管毒性类型心律失常、心包疾病;多发生于用药过程中或用药后几天内,大多数药物相关心血管毒性转归良好,有导致死亡的报道。结论:紫杉烷类抗肿瘤药物相关心血管毒性临床常见,大多数预后较好,需警惕致死风险。Objective:To investigate the cardiovascular toxicity of taxanes. Methods: Cardiovascular toxicity of taxanes was retrieved in National Adverse Drug Reaction Monitoring System, as related literature was collected from CNKI, Wanfang, PubMed, and Emabse databases. Types of cardiovascular toxicity, occurrence time, outcome and influencing factors were analyzed and summarized. Results: A Total of 75 cases were enrolled, among which cardiovascular toxicity regarding paclitaxel, docetaxel, albumin-bound paclitaxel, and paclitaxel liposome were 43, 26, 5, and 1, respectively. The top 3 types of cardiovascular toxicity of taxanes were arrhythmia, hypertension, and coronary artery disease;the most common ones of docetaxel were arrhythmia and pericardial complications. The cardiovascular toxicity occurred during the treatment or within a few days after medication. Most cases of cardiovascular toxicity were reversible;however, reports of death were reported. Conclusion: The cardiovascular toxicity of taxanes is common in clinical practice and the most prognosis is satisfactory, but it is necessary to be alert to the risk of death caused by taxanes.
