原发性高血压病左心室肥厚与动脉硬化指标的关系研究

摘　　要：目的探讨原发性高血压病左心室肥厚(LVH)与动脉硬化指标的关系。方法选取福建医科大学附属第二医院2014—2015年收治的原发性高血压病患者107例,根据左心室质量指数(LVMI)分为LVH组(n=42)和非LVH组(n=65)。比较2组年龄、性别、体质指数(BMI)、空腹血糖(FBG)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、血尿酸(UA)、血肌酐(Scr)、收缩压(SBP)、舒张压(DBP)、心率(HR)、颈动脉内中膜厚度(CIMT)、臂踝脉搏波传导速度(baPWV),分析LVMI与CIMT、baPWV的相关性。结果LVH组年龄、CIMT、baPWV高于非LVH组(P<0.05)。2组性别、BMI、FBG、TG、TC、HDL、LDL、UA、Scr、SBP、DBP、HR比较,差异无统计学意义(P>0.05)。Pearson相关分析结果显示,LVMI与CIMT、baPWV、年龄、FBG、TC、UA呈正相关(P<0.05),LVMI与BMI、TG、HDL、LDL、Scr、HR无相关性(P>0.05)。以LVMI为因变量,以CIMT、baPWV、年龄、FBG、TC、UA为自变量进行多元线性回归分析,结果显示,CIMT、FBG、TC、UA是LVMI的影响因素(P<0.05)。结论原发性高血压病LVH与CIMT、baPWV相关,CIMT是LVMI的影响因素。Objective To explore the relationship between left ventricular hypertrophy(LVH)and arteriosclerosis indexes in essential hypertension.Methods A total of 107 cases of patients with essential hypertension were selected from 2014 to 2015 in the Second Affiliated Hospital of Fujian Medcial University,which were divided into LVH group(n=42)and non-LVH group(n=65)according to the left ventricular mass index(LVMI).Age,gender,BMI,FBG,TC,TG,HDL,LDL,UA,Scr,SBP,DBP,HR,CIMT,baPWV were compared between the two groups,to analyze the correlation between LVMI and CIMT and baPWV.Results The age,CIMT and baPWV in LVH group were higher than non-LVH group(P<0.05).There was no significant difference in gender,BMI,FBG,TG,TC,HDL,LDL,UA,Scr,SBP,DBP and HR between the two groups(P>0.05).Pearson correlation analysis showed that LVMI was positively correlated with CIMT,baPWV,age,FBG,TC and UA(P<0.05),LVMI had no correlation with BMI,TG,HDL,LDL,Scr and HR(P>0.05).With LVMI as the dependent variable and CIMT,baPWV,age,FBG,TC,UA as the independent variables,the results show that CIMT,FBG,TC,UA are the influencing factors of LVMI(P<0.05).With LVMI as the dependent variable and CIMT,baPWV,age,FBG,TC,UA as the independent variables,the results show that CIMT,FBG,TC,UA ere the influencing factors of LVMI(P<0.05).Conclusion LVH of essential hypertension is related to CIMT and baPWV,and CIMT is the influencing factor of LVMI.
