血清降钙素原与B型尿钠肽在感染性休克患者体液复苏治疗前后的意义

摘　　要：目的探讨ICU感染性休克患者体液复苏治疗前后对患者进行血清降钙素原(PCT)与B型尿钠肽(BNP)检测的价值。方法选取该院ICU在2018年2月—2020年2月期间接收的130例感染性休克患者,入院后均监测检查患者的动脉血气结果、24 h累计复苏治疗液体总量及复苏达标的时间,同时检测体液复苏治疗前后的血清降钙素原与B型尿钠肽水平。结果治疗后PH值(7.35±0.01)、Na+(142.20±4.35)mmol/L、CI-(115.62±4.62)mmol/L、PCT水平(9.87±2.53)ng/mL、BNP水平(237.43±60.34)ng/mL与治疗前比较差异有统计学意义(t=5.833、-4.082、-4.281、6.835、20.533,P<0.05);复苏24 h患者血清PCT水平与复苏液体总量之间呈现负相关(r=-0.934,P<0.05)。结论血清PCT与BNP的改变水平和感染性休克患者病情之间关系密切,且两者指标可作为评估该疾病治疗效果的监测指标。Objective To explore the value of serum procalcitonin(PCT)and B-type natriuretic peptide(BNP)in patients with septic shock in ICU before and after resuscitation.Methods A total of 130 patients with septic shock received in the ICU of the hospital from February 2018 to February 2020 were selected.After admission,the patients’arterial blood gas results,the total amount of resuscitation treatment fluid in 24 h and the time to reach the resuscitation standard were monitored and checked.The levels of serum procalcitonin and B-type natriuretic peptide before and after the body fluid resuscitation treatment were detected.Results After treatment,the PH value(7.35±0.01),Na+(142.20±4.35)mmol/L,CI-(115.62±4.62)mmol/L,PCT level(9.87±2.53)ng/mL,BNP level(237.43±60.34)ng/mL were compared with those before treatment;the difference was statistically significant(t=5.833,-4.082,-4.281,6.835,20.533,P<0.05);there was a negative correlation between the serum PCT level and the total amount of resuscitation fluid(r=-0.934,P<0.05).Conclusion There is a close relationship between the change levels of serum PCT and BNP and the condition of patients with septic shock,and the two indicators can be used as monitoring indicators to evaluate the treatment effect of the disease.
