摘 要:目的探析慢性肾脏病(CKD)患者钙磷代谢紊乱对心血管事件的影响。方法将该院2015年1月—2019年12月收治的60例CKD患者根据疾病分期分成CKD2期、CKD3期、CKD4期、CKD5期;根据是否发生心血管事件分成甲组(未发生心血管事件)与乙组(发生心血管事件)。检测对比各组患者血钙、血磷、临床指标、实验室指标水平,采用Logistic多因素回归分析CKD患者发生心血管事件的独立危险因素。结果CKD5期血钙(2.06±0.11)mmol/L,明显低于CKD2期、CKD3期、CKD4期,差异有统计学意义(t=10.280、9.250、8.120,P<0.05);血磷(1.61±0.19)mmol/L,明显高于CKD2期、CKD3期、CKD4期,差异有统计学意义(t=8.230、10.120、8.670,P<0.05)。甲组平均年龄、既往糖尿病病史、既往冠心病病史、接受血液透析治疗为(59.56±5.35)岁、12.20%、58.54%、21.95%,乙组(66.27±5.43)岁、36.84%、89.47%、57.89%,差异有统计学意义(t=7.360,χ2=4.650、6.270、5.830,P<0.05)。甲组血钙、血磷、血肌酐、总胆固醇分别为(2.21±0.13)mmol/L、(1.27±0.12)mmol/L、(423.38±56.72)μmol/L、(4.56±0.89)mmol/L,乙组(2.06±0.11)mmol/L、(1.54±0.18)mmol/L、(605.37±59.43)μmol/L、(5.49±0.91)mmol/L,差异有统计学意义(t=6.24、5.120、7.680、8.130,P<0.05)。结论CKD患者普遍存在低钙、高磷的现象,且低钙是患者发生心血管事件的独立危险因素。Objective To explore the influence of disorders of calcium and phosphorus metabolism on cardiovascular events in patients with chronic kidney disease(CKD).Methods The 60 CKD patients admitted to the hospital from January 2015 to December 2019 were divided into CKD stage 2,CKD stage 3,CKD stage 4,and CKD stage 5 according to the disease stage;they were divided into group A according to whether a cardiovascular event occurred(no cardiovascular event occurred)and group B(a cardiovascular event occurred).The levels of blood calcium,blood phosphorus,clinical indicators,and laboratory indicators in each group were detected and compared.Logistic multivariate regression was used to analyze independent risk factors for cardiovascular events in CKD patients.Results CKD stage 5 blood calcium(2.06±0.11)mmol/L,significantly lower than CKDstages 2,CKDstages 3,CKDstages 4 stages,the difference was statistically significant(t=10.280,9.250,8.120,P<0.05);blood phosphorus(1.61±0.19)mmol/L,significantly higher than CKDstages 2,CKDstages 3,CKDstages 4 stages,the difference was statistically significant(t=8.230,10.120,8.670,P<0.05).The average age of group A,past history of diabetes,past history of coronary heart disease,and hemodialysis treatment were(59.56±5.35)years,12.20%,58.54%,21.95%,and group B(66.27±5.43)years old,36.84%,89.47%,57.89%,the difference was statistically significant(t=7.360,χ2=4.650,6.270,5.830,P<0.05).The blood calcium,blood phosphorus,blood creatinine,and total cholesterol of group A were(2.21±0.13)mmol/L,(1.27±0.12)mmol/L,(423.38±56.72)μmol/L,(4.56±0.89)mmol/L,respectively,group B(2.06±0.11)mmol/L,(1.54±0.18)mmol/L,(605.37±59.43)μmol/L,(5.49±0.91)mmol/L,the difference was statistically significant(t=6.240,5.120,7.680,8.130,P<0.05).Conclusion CKD patients generally have low calcium and high phosphorus,and low calcium is an independent risk factor for cardiovascular events in patients.
