基于聚类分析的杨晋翔教授治疗反流性食管炎的用药规律及学术思想

摘　　要：目的:探究在治疗反流性食管炎方面杨晋翔教授的用药经验和配伍规律并总结其学术思想。方法:收集2016年12月至2018年1月在门诊期间确诊为反流性食管炎且经杨晋翔教授治疗的RE患者156例,采集其首诊中药处方作为研究对象。将其录入Excel软件中并运用频数分析的方法筛选出高频药物,并根据功效进行分类;基于“中医传承辅助平台”软件中聚类分析的方法挖掘出所有处方中的常用核心处方和药对,总结出杨晋翔教授的学术思想。结果:所有处方中各中药的总频数位于首位的是青蒿,徐长卿、吴茱萸、黄连的使用频数同样较高;清热药在高频中药功效分类中位于首位,依次是利湿、化痰以及活血化瘀药;聚类分析的结果显示出杨晋翔教授在治疗反流性食管炎方面常用的5个核心处方和6个常配伍运用的核心药对。结论:杨晋翔教授治疗反流性食管炎以清郁热、除痰湿、调气血为主要治法,反映了其虚实同理、邪正兼顾、气血并施的学术思想。Objective:To explore Professor YANG Jinxiang′s medication experience and compatibility rules in the treatment of reflux esophagitis and summarize his academic thoughts.Methods:Patients who were diagnosed with reflux esophagitis during the outpatient period from December 2016 to January 2018 were collected and were treated by Professor YANG Jinxiang.Their first Chinese medicine prescriptions were collected.We entered them into Excel software and used the frequency analysis method to screen out high-frequency drugs,and classified them according to their efficacy;based on the cluster analysis method in the“Traditional Chinese Medicine Inheritance Auxiliary Platform”software,the common core prescriptions and drug pairs in all prescriptions were mined,and the academic thoughts of Professor YANG Jinxiang summarized.Results:The drug frequency ranked the top 4 was Herba Artemisiae Annuae,Radix Cynanchi Paniculati,Fructus Evodiae,Rhizoma Coptidis.The main types of high-frequency drugs were heat-clearing drugs,hydrophilic and hygroscopic drugs,phlegm-resolving drugs and blood-activating and blood-stasis drugs.Cluster analysis revealed 5 core prescriptions and 6 common drug pairs.Conclusion:Professor YANG Jinxiang treats reflux esophagitis by clearing away heat,removing phlegm and dampness and regulating qi and blood.It reflects his academic thoughts of differentiation between deficiency and excess,the combination of pathogen and right qi,combination of qi and blood.
