便携式睡眠监测仪对阻塞性睡眠呼吸暂停低通气综合征诊断的方便性研究

摘　　要：目的探究便携式睡眠监测仪(PMDs)对诊断阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的方便性。方法进行睡眠监测调查者118例。根据患者意愿选择睡眠监测方式分为干预组(n=70)和对照组(n=48)。干预组使用PMDs,对照组使用标准多导睡眠监测(PSG)。比较2组调查对象的睡眠监测成功率,各项睡眠监测参数、PMDs检查灵敏度及特异度和临床满意度。结果干预组监测成功率高于对照组,差异有统计学意义(P<0.05);2组调查对象的总睡眠监测时间、最低血氧饱和度和平均血氧饱和度的数值差异无统计学意义(P>0.05);干预组PMDs诊断灵敏度98.30%,特异性57.14%;干预组临床满意度高于对照组,差异有统计学意义(P<0.05)。结论PMDs灵敏度高,可以显著提高OSAHS患者的监测成功率,与传统检查方式得到的多项睡眠参数无明显差异,且临床满意度高,为OSAHS早期发现提供思路和视野。Objective To investigate the the convenience of portable sleep monitor(PMDs)in diagnosis of obstructive sleep apnea hypopnea syndrome(OSAHS).Methods A total of 118 patients with OSAHS who underwent sleep monitoring survey in our hospital from February 2016 to January 2017 were enrolled in the study,who were divided into intervention group(n=70)and control group(n=48)according to the wishes of patients.The patients in intervention group were monitored by using PMDs,however,the patients in control group weremonitored by using standard polysomnography(PSG).The sleep monitoring success rate,sleep monitoring parameters,PMDs examination sensitivity and specificity,and clinical satisfaction degree were observed and compared between the two groups.Results The monitoring success rate in intervention group was 94.3%,which was significantly higher than that(81.3%)in control group(P<0.05).There were no significant differences in the total sleep monitoring time,the minimum blood oxygen saturation and the average blood oxygen saturation between the two groups(P>0.05).Moreover the diagnostic sensitivity of PMDs in intervention group was 98.30%and the specificity was 57.14%,and the clinical satisfaction degree in intervention group was 97.14%,which was significantly higher than that 43.75%in control group(P<0.05).Conclusion The portable sleep breathing monitor has higher sensitivity,which can significantly improve the monitoring success rate of patients with OSAHS.There are no significant differences in many sleep parameters between PMDs and traditional examination way,with higher clinical satisfaction degree,which provides ideas and vision for early discovery of OSAHS.
