开环式吻合器在老年脱垂性痔病的临床研究

摘　　要：目的研究开环式吻合器在老年脱垂性痔病的临床疗效。方法回顾分析2017年1月-2019年1月在该院诊治的100例脱垂性痔病患者,随机分为对照组和观察组,每组50例。对照组采用吻合器痔上黏膜环切术(PPH)治疗,观察组采用开环式微创肛肠吻合器痔切闭术(TST)治疗,对比两组临床治疗疗效、手术情况、术后疼痛评分、以及并发症发生率。结果观察组手术时间(24.5±3.8)min、住院时间(5.7±1.0)d短于对照组(32.1±4.7)min、(8.2±1.3)d,而术后疼痛评分(3.5±0.8)分低于对照组(5.3±1.3)分,差异有统计学意义(t=8.338、10.778、8.338,P<0.05);观察组总有效率98.0%明显高于对照组总有效率84.0%,差异有统计学意义(χ^2=4.396,P<0.05);对照组并发症发生率38.0%,观察组并发症发生率12.0%,观察组低于对照组(χ^2=9.013,P<0.05)。结论老年脱垂性痔病采用TST术治疗效果显著,手术时间短,术后出血发生率低、疼痛轻,且肛门水肿、尿潴留等并发症发生率低,值得临床推广和应用。Objective To study the clinical efficacy of open-loop stapler in senile prolapsed hemorrhoids.Methods A retrospective analysis of 100 patients with prolapsed hemorrhoids diagnosed and treated in the hospital from January 2017 to January 2019 were randomly divided into a control group and an observation group,with 50 cases in each group.The control group was treated with stapled prolapse and hemorrhoidectomy(PPH),and the observation group was treated with open loop minimally invasive anorectal stapler hemorrhoidectomy(TST).The two groups were compared with the clinical treatment effect,operation status,and postoperative pain score and complication rate.Results The observation group’s operation time(24.5±3.8)min and hospitalization time(5.7±1.0)d were shorter than those of the control group(32.1±4.7)min,(8.2±1.3)d,while the postoperative pain score(3.5±0.8)points was lower than the control group(5.3±1.3)points,and the differences were statistically significant(t=8.338,10.778,8.338,P<0.05);the total effective rate of the observation group was 98.0%significantly higher than that of the control group of 84.0%,and the difference was statistically significant(χ^2=4.396,P<0.05);the complication rate of the control group was 38.0%,the complication rate of the observation group was12.0%,and the observation group was lower than the control group(χ^2=9.013,P<0.05).Conclusion TST surgery for senile prolapsed hemorrhoids has a significant effect,short operation time,low incidence of postoperative bleeding,low pain,and low incidence of complications such as anal edema and urinary retention.It is worthy of clinical promotion and application.
