持续气管滴液联合机械振动排痰在气管切开患者预防痰栓护理效果

摘　　要：目的持续气管滴液联合机械振动排痰在气管切开患者预防痰栓护理效果。方法方便选取该院于2018年1-12月收治的108例气管切开患者,采用电脑随机法将其划分为观察组与对照组,各54例,观察组采取持续气管滴液联合机械振动排痰护理,对照组单独应用机械振动排痰护理,评价两组患者疗效、痰栓个数、血氧饱和度、吸痰次数、肺部感染发生率。结果观察组患者治疗总有效率96.30%,高于对照组81.48%(χ^2=6.000,P<0.05);观察组痰栓个数2个、低于对照组8个(χ^2=3.967,P<0.05)。干预前两组血氧饱和度、吸痰次数结果比较差异无统计学意义(P>0.05);干预后观察组血氧饱和度(97.32±1.09)%、吸痰次数(10.82±1.83)次/d,均优于对照组(94.25±1.22)%、(14.26±2.06)次/d(P<0.05)。观察组肺部感染发生率1例(1.85%),低于对照组8(14.81%)(χ^2=4.364,P<0.05)。结论气管切开患者采取持续气管滴液联合机械振动排痰可有效预防痰栓、提高疗效、改善血氧饱和度、吸痰次数,预防肺部感染发生率。Objective The effect of continuous tracheal drip combined with mechanical vibration expectoration to prevent phlegm thrombus in patients with tracheotomy.Methods 108 patients with tracheotomy admitted to the hospital from January to December 2018 were conveniently selected,and they were divided into observation group and control group with54 cases each by computer randomization.The observation group was treated with continuous tracheal drip combined with mechanical vibration Sputum expectoration nursing.The control group used mechanical vibration expectoration nursing alone to evaluate the efficacy,the number of sputum thrombus,blood oxygen saturation,the number of sputum suctions,and the incidence of pulmonary infection in the two groups.Results The total effective rate of treatment in the observation group was 96.30%,which was higher than 81.48%in the control group(χ^2=6.000,P<0.05);the number of sputum plugs in the observation group was 2,which was lower than the control groups 8(χ^2=3.967,P<0.05).There was no statistically significant difference in blood oxygen saturation and the number of sputum suctions between the two groups before the intervention(P>0.05);after the intervention,the blood oxygen saturation of the observation group was(97.32±1.09)%and the number of sputum suctions was(10.82±1.83)times/d.All were better than the control group(94.25±1.22)%,(14.26±2.06)times/d(P<0.05).The incidence of lung infection in the observation group was 1 case(1.85%),which was lower than 8 cases(14.81%)in the control group(χ^2=4.364,P<0.05).Conclusion The use of continuous tracheal drops combined with mechanical vibration for sputum excretion in patients with tracheotomy can effectively prevent sputum thrombosis,improve curative effect,improve blood oxygen saturation,the frequency of sputum suction,and prevent the incidence of pulmonary infection.
