经尿道膀胱颈电切术联合经尿道柱状水囊扩张术治疗TURP术后膀胱颈挛缩疗效观察

摘　　要：目的观察经尿道膀胱颈电切术联合经尿道柱状水囊扩张术在治疗经尿道前列腺电切术(TURP)术后膀胱颈挛缩(BNC)的效果。方法选择2017年2月~2018年10月,因TURP术后出现排尿困难,经膀胱镜及最大尿流率等相关检查后,诊断为膀胱颈挛缩的患者9例。术前统计患者最大尿流率(Qmax)、国际前列腺评分(IPSS)及生活质量评分(QOL),行手术治疗后统计术后3个月及6个月Qmax,IPSS评分,QOL评分,并与术前结果进行对比。结果术后3个月、6个月的Qmax,IPSS评分,QOL评分较术前均有明显改善,差异均有显著性(P<0.01)。结论经尿道膀胱颈电切术联合经尿道柱状水囊扩张术可改善TURP术后膀胱颈挛缩患者的症状,提高生活质量。Objective To observe the clinical efficacy of transurethral resection of bladder neck combined with transurethral cylindrical balloon dilatation in the treatment of bladder neck contracture after TURP.Methods From February 2017 to October 2018,9 patients with bladder neck contracture were diagnosed by cystoscopy and maximum urinary flow rate because of dysuria after TURP.The maximum urinary flow rate(Qmax),international prostate score(IPSS) and quality of life score(QOL) were calculated before operation.The Qmax,IPSS and QOL were calculated 3 and 6 months after operation seperately,and compared with the preoperative results.Results The Qmax,IPSS and QOL at 3 and 6 months after operation were significantly improved compared with those before operation,which has statistically significant difference(P<0.01).Conclusion Transurethral resection of bladder neck combined with transurethral cylindrical balloon dilatation can relief the symptoms and improve the life quality of patients with bladder neck contracture after TURP.
