健脾养胃法治疗胃癌作用及机制研究进展

摘　　要：胃癌是一种危害极大的常见恶性肿瘤,具有发病率高、转移率高、死亡率高,早期诊断率低、根治性切除低、五年生存率低的特点。中医药在其防治中具有独特的优势。按照中医理论,脾胃虚弱是胃癌发生的主要内因,健脾养胃是治疗胃癌的治本之法。本文通过检索中国知网、万方、PubMed等数据库,搜集整理国内外有关健脾养胃法治疗胃癌的研究报道,综述健脾养胃法在防治胃癌上的研究进展。发现以健脾养胃法为组方核心的方药在抗胃恶性肿瘤方面,具有多靶点、多层次的作用特点。其机制可能与抑制胃癌细胞增殖,诱导胃癌细胞凋亡,分化和抑制胃癌细胞侵袭与转移,抗肿瘤血管生成,抑制胃癌信号通路,提高机体免疫力,调节肿瘤免疫微环境,逆转胃癌化疗耐药和肠道菌群失调等密切相关。Gastric cancer is a common malignant tumor with great harm.It has the characteristics of high incidence rate,high metastasis rate,high mortality rate,low early diagnosis rate,low radical resection rate and low five year survival rate.Traditional Chinese medicine has unique advantages in its prevention and treatment.According to the theory of traditional Chinese medicine,weakness of the spleen and stomach is the main internal cause of gastric cancer,and invigorating spleen and nourishing stomach is the fundamental method in the treatment of gastric cancer.By retrieving the databases of CNKI,Wanfang,PubMed and others in this paper,the research reports on the treatment of gastric cancer by the method of invigorating spleen and nourishing stomach at home and abroad were collected and sorted out,and the research progress on the prevention and treatment of gastric cancer by the method of invigorating spleen and nourishing stomach was reviewed.It is found that the core prescription of invigorating spleen and nourishing stomach has the characteristics of multi-target and multi-level action in anti-gastric cancer.The mechanism may be closely related to inhibiting proliferation of gastric cancer cell,inducing its cell apoptosis and differentiation,inhibiting invasion and metastasis of gastric cancer cell,resisting the generation of tumor angiogenesis,inhibiting signaling pathway of gastric cancer,improving body immunity,regulating immune microenvironment of tumor,reversing chemotherapy resistance of gastric cancer and disorder of intestinal flora.
