摘 要:目的探讨高海拔阻塞性睡眠呼吸暂停低通气综合征(OSAHS)藏汉族患者血清缺氧诱导因子-2α(HIF-2α)的水平及意义。方法选取2017~2019年于青海大学附属医院体检正常及确诊OSAHS的藏汉族,分为4组:移居高海拔>3年的健康(Ⅰ)和患病(Ⅱ)汉族,世居高海拔的健康(Ⅲ)和患病(Ⅳ)藏族。对受试者进行多导睡眠监测,用ELISA检测血清HIF-2α水平,分析其与监测指标及血红蛋白(Hb)相关性。结果组Ⅱ和组Ⅳ的呼吸紊乱指数(AHI)、晨起收缩/舒张压、血氧饱和度<90%的时间、Hb及HIF-2α水平较组Ⅰ和组Ⅲ升高(t=16.483、12.158、19.234、22.342、10.186、9.584;t=28.123、14.634、16.583、19.563、30.256、20.183,P<0.05),最低血氧饱和度(MinSaO 2)较组Ⅰ和组Ⅲ降低(t=17.435、t=35.624,P<0.05)。组Ⅲ和组ⅣHb含量较组Ⅰ和组Ⅱ下调(t=26.347;t=10.434,P<0.05)。组Ⅱ和组ⅣHIF-2α水平与AHI呈正相关(r=0.4815、0.5283,P<0.05),与MinSaO 2呈负相关(r=-0.5847、-0.4997,P<0.05)。结论高海拔OSAHS藏汉族患者血清HIF-2α水平高于正常藏汉族,HIF-2α水平与OSAHS的严重程度成正相关。Objective To investigate the level and significance of serum hypoxia-inducible factor-2α(HIF-2α)in high-altitude Tibetan and Han people with obstructive sleep apnea-hypopnea syndrome(OSAHS).Methods The Tibetan and Han people with normal physical examinations and those with OSAHS diagnosed in the Affiliated Hospital of Qinghai University from 2017 to 2019 were selected and divided into 4 groups:healthy(the groupⅠ)and sick(the groupⅡ)Han people who migrated to high altitude>3 years,and healthy(the groupⅢ)and sick(the groupⅣ)Tibetans living at high altitudes for generations.The subjects were monitored for polysomnography,and the serum HIF-2αlevels were detected by ELISA,and the correlations between the serum HIF-2αand monitoring indexes or hemoglobin(Hb)were analyzed.Results In the groupⅡand the groupⅣ,their Apnea Hyponea Index(AHI),morning systolic and diastolic blood pressure,the time of blood oxygen saturation less than 90%(T-SaO 2<90%),Hb content and HIF-2αlevels were up-regulated(t=16.483,12.158,19.234,22.342,10.186,9.584;t=28.123,14.634,16.583,19.563,30.256,20.183,P<0.05),and the minimum blood oxygen saturation(MinSaO 2)was down-regulated(t=17.435;t=35.624,P<0.05),respectively compared with the group I and the groupⅢ.The levels of Hb in the groupⅢand the groupⅣwere lower than those in the group I and the groupⅡ(t=26.347;t=10.434,P<0.05).The HIF-2αlevels in the groupⅡand the groupⅣwere positively correlated with AHI(r=0.4815,0.5283,P<0.05),and negatively correlated with MinSaO 2(r=-0.5847,-0.4997,P<0.05).Conclusion The HIF-2αlevel in high altitude OSAHS Tibetan and Han patients is higher than that in normal high-altitude Tibetan and Han people,and the level of HIF-2αis positively correlated with the severity of OSAHS.
