血清tIgE及肺功能在重症哮喘及过敏性鼻炎患者中临床应用价值

摘　　要：目的探讨血清tIgE水平及肺功能在重症哮喘及同时合并有过敏性鼻炎患者中临床应用价值。方法回顾性分析2019年8月-2019年11月符合入组标准的住院患者104例相关临床资料,t检验显示外周血tIgE高水平与重症哮喘同时合并过敏性鼻炎的发生相关(P<0.05);肺功能指标一秒率(FEV 1/FVC)及25%~75%之间用力肺活量时的呼吸流速(FEF 25%-75%)占预计值百分比水平,在重症哮喘同时合并过敏性鼻炎的更低(P<0.05)。χ2检验显示患者的年龄与重症哮喘同时合并有过敏性鼻炎的发生无关(P>0.05)。绘制tIgE的ROC生存曲线,ROC曲线下面积为0.868,得出tIgE水平在预测重症哮喘合并过敏性鼻炎的发生中具有较高的价值(灵敏度78.0%,特异度92.1%)。结论在重症哮喘合并过敏性鼻炎的患者中,血清tIgE水平比单纯重症哮喘组高,肺功能更差,此发现可以指导重症哮喘的升降阶梯治疗,及时治疗鼻炎有助于降低重症哮喘合并鼻炎患者的tIgE水平,tIgE浓度对重症哮喘患者中筛查鼻炎有诊断性意义。Objective To investigate the clinical value of serum tIgE level and lung function in patients with severe asthma and allergic rhinitis.Methods The clinical data of 104 inpatients who met the inclusion criteria from August 2019 to November 2019 were retrospectively analyzed.Results The t-test showed that high tIgE level in peripheral blood was correlated with the incidence of severe asthma complicated with allergic rhinitis(P<0.05).FEV 1/FVC level and respiratory velocity(FEF 25%-75%)at forced vital capacity between 25%and 75%as a percentage of the expected value were lower in severe asthma combined with allergic rhinitis(P<0.05).The test 2 showed that the age of the patients had nothing to do with the occurrence of severe asthma combined with allergic rhinitis(P>0.05).The ROC survival curve of tIgE was drawn.The area under the ROC curve was 0.868 and it was concluded that tIgE level was of high value in predicting the occurrence of severe asthma complicated with allergic rhinitis(sensitivity 78%,specificity 92.1%).Conclusion In patients with severe asthma complicated with allergic rhinitis,the serum tIgE level is higher and the lung function is worse than that of the patients with severe asthma alone,which can guide the ladder treatment of severe asthma.Timely treatment of rhinitis can help reduce IgE levels in patients with severe asthma complicated with rhinitis,and the IgE gradient has diagnostic significance in screening rhinitis in patients with severe asthma.
