摘　　要：总结马融教授治疗小儿难治性哮喘的临证经验。认为小儿难治性哮喘发作期和缓解期不应按照是否有喘息发作而截然分开,缓解期患儿即使无明显喘息症状,但如肺功能异常也可以按照发作期辨治。小儿难治性哮喘病程日久,也有实邪因素存在,所以治疗时要标本同治。根据患儿的临床症状,提出了病程中的"气闭""痰阻""血瘀""肺痉""肾虚"五大病理因素,并分别采用麻黄、炒苦杏仁、蜜桑白皮、煅赭石、白果等宣肃开闭;瓜蒌、清半夏、细辛等涤痰逐饮;郁金、当归、红花等活血化瘀;地龙、炒僵蚕、钩藤等祛风解痉;熟地黄、阿胶、肉桂等补肾固本。To summarize the clinical experience of professor MA Rong in the treatment of with refractory asthma in children.Professor MA Rong thinks children with refractory asthma episode and remission should not be separated according to whether there is asthma attack.Even there is no obvious symptoms of wheezing,patients can also be treated in accordance with the onset period if pulmonary function show abnormal.Although the course of the disease is long,there is excessive pathogen.Thus root cause and symptom treatment should be applied.According to the clinical symptoms of child patients,the five major pathological factors of qi blockage,phlegm obstruction,blood stasis,lung spasm and kidney deficiency in the course of the disease are proposed.Mahuang(Herba Ephedrae),stir-frying Ku Xingren(Semen Armeniacae Amarum),Sangbaipi(Cortex Mori)stir-frying with honey,Zheshi(calcining Haematitum),Baiguo(Semen Ginkgo)are used to ventilating lung and to open block;Gualou(Fructus Trichosanthis),Qing Banxia(Rhizoma Pinelliae Preparata),Xixin(Herba Asari)are used to expel phlegm and fluid retention;Yujin(Radix Curcumae),Danggui(Radix Angelicae Sinensis),Honghua(Flos Carthami)are used to promote blood circulation for removing blood stasis;Dilong(Lumbricus),stir-frying Jiangcan(Bombyx Batryticatus),Gouteng(Ramulus Uncariae Cum Uncis)are used to dispel pathogenic wind for resolving convulsion;Shudihuang(Radix Rehmanniae Preparata),Ajiao(Colla Corii Asini),Rougui(Cortex Cinnamomi)are used to tonify the kidney and consolidate the resistance.
