摘 要:目的探讨中年男性静息心率(RHR)、最大摄氧量(VO2max)及颈动脉内膜中层厚度(CIMT)与心血管危险因素的关系。方法回顾性收集2014年3月~2019年3月于成都体育学院附属体育医院进行体检的506例中年男性的临床资料。根据RHR将患者分为4个水平(<60bpm、60~69bpm、70~79bpm、≥80bpm)进行分析,根据VO2max将患者分为低(≤33.3%)、中(>33.3%,≤66%)、高(>66.6%)3个水平进行分析。采用标准的12导联自动心脏换能器测量RHR,采用自动呼吸气体分析系统确定VO2max,采用彩色多普勒超声诊断仪测量颈动脉内膜中层厚度。分析RHR、VO2max、CIMT与心血管危险因素的相关性。结果RHR≥80bpm患者VO2max较RHR<60bpm患者显著降低(P<0.05)。RHR≥80bpm患者收缩压、舒张压、TG、FBG较RHR<60bpm患者显著升高(P<0.05)。不同RHR患者间颈动脉内膜中层厚度(CIMT)比较差异无统计学意义(P>0.05)。高VO2max患者RHR、TG、FBG、ALT较低VO2max患者均显著降低(P<0.05)。中、高VO2max患者γ-GTP、CIMT较低VO2max患者均显著降低(P<0.05)。RHR与收缩压、舒张压、TG、FBG、γ-GTP呈正相关(P<0.05)。VO2max与BMI、收缩压、舒张压、T-Chol、TG、LDL-C、FBG、ALT、γ-GTP、CIMT呈负相关(P<0.05)。低VO2max患者发生颈动脉粥样硬化的相对风险是高VO2max患者的3.564倍(95%CI:1.773~7.166)。结论VO2max与BMI、收缩压、舒张压、T-Chol、TG、LDL-C、FBG、ALT、γ-GTP、CIMT呈负相关,检测结果可作为预测颈动脉粥样硬化发病风险的重要指标。Objective To investigate the relationship between resting heart rate(RHR)and maximal oxygen uptake(VO2max)and carotid intima-media thickness in middle-aged men.Methods The clinical data of 506 middle-aged men came to our hospital for medical examination from March 2014 to March 2019 were collected.The patients were divided into 4 levels(<60bpm,60-69bpm,70-79bpm,≥80bpm)according to the resting heart rate.The patients were divided into low(≤33.3%),medium(>33.3%,≤66%)and high(>66.6%)3 levels for analysis according to the maximum oxygen uptake.RHR was measured using a standard 12-lead automatic heart transducer,VO2max was determined using an automated respiratory gas analysis system,and carotid intima-media thickness was measured using a color Doppler ultrasound system.Results The VO2max of patients with RHR≥80bpm was significantly lower than that of patients with RHR<60bpm(P<0.05).Systolic blood pressure,diastolic blood pressure,TG and FBG were significantly higher in patients with RHR≥80bpm than those with RHR<60bpm(P<0.05).There was no significant difference in carotid intima-media thickness(CIMT)between patients with different RHR(P>0.05).Patients with high VO2max had significantly lower RHR,TG,FBG,and ALT with lower VO2max(P<0.05).Patients with moderate to high VO2max had significantly lowerγ-GTP and CIMT lower VO2max(P<0.05).There was a significant positive correlation between RHR and systolic blood pressure,diastolic blood pressure,TG,FBG andγ-GTP(P<0.05).There was a significant negative correlation between VO2max and BMI systolic blood pressure,diastolic blood pressure,T-Chol,TG,LDL-C,FBG,ALT,γ-GTP and CIMT(P<0.05).The relative risk of carotid atherosclerosis in patients with low VO2max was 3.564 times higher than the patients with high VO2max(95%CI:1.773-7.166).Conclusion VO2max was significantly negatively correlated with BMI,systolic blood pressure,diastolic blood pressure,T-Chol,TG,LDL-C,FBG,ALT,γ-GTP,and CIMT.In addition,VO2max was an important indicator for predicting the risk of carotid a
