摘　　要：目的探讨成人腮腺Warthin's瘤的超声造影声像表现,观察其超声造影参数变化。方法32例成人腮腺Warthin's瘤患者,入院二维超声检查共发现34个肿块,手术治疗前均行超声造影检查。记录成人腮腺Warthin's瘤超声造影特征;绘制时间-强度曲线,以34个肿块为观察组,肿块周围正常腮腺组织为对照组,比较2组达峰时间、峰值强度、峰值半降时间、曲线下面积。结果34个肿块超声造影均表现为等进慢出不均匀高增强,其中31个肿块增强后边界清晰、可见完整增强环,3个囊性变结节内部可见少许不规则无增强区。时间-强度曲线显示,观察组达峰时间[(16.59±2.29)s]与对照组[(16.26±2.60)s]比较差异无统计学意义(P>0.05),峰值强度[(19.95±3.27)dB]高于对照组[(9.95±2.49)dB]、峰值半降时间[(57.94±7.57)s]长于对照组[(41.03±4.55)s]、曲线下面积[(4263.97±426.06)dB.s]大于对照组[(1920.42±125.37)dB.s](P<0.05)。结论腮腺Warthin's瘤超声造影主要表现为边界清晰、等进慢出高增强,增强环完整。Objective To investigate the contrast-enhanced ultrasonography(CEUS)findings of adult Warthin's tumor and to observe the changes of CEUS parameters.Methods Thirty-two adult patients with Warthin's tumor were detected 34tumors on two-dimensional ultrasound at admission,and all received CEUS before operation.The CEUS features of adult Warthin's tumors were recorded and the time-intensity curve was drawn.The time to peak,peak intensity,time from peak to one half and area under the curve were compared between 34tumors(observation group)and the adjacent normal parotid glands(control group).Results The CEUS of 34tumors showed“identical in slow out”and inhomogeneous hyperenhancement,in which 31tumors showed well-defined boundary and complete enhancement rim,and the other 3cystic nodules showed a few irregular areas without enhancement.The time-intnsity curve showed no significant difference in the time to peak between two groups((16.59±2.29)s vs.(16.26±2.60)s)(P>0.05).The peak intensity was higher,the time from peak to one half was longer and the area under the curve was larger in observation group((19.95±3.27)dB,(57.94±7.57)s,(4263.97±426.06)dB.s)than that in control group((9.95±2.49)dB,(41.03±4.55)s,(1920.42±125.37)dB.s)(P<0.05).Conclusion The CEUS of Warthin's tumor shows clear boundary,“identical in slow out”,hyperenhancement and complete enhancement rim.
