摘  要:目的应用静息态功能磁共振成像(rs-fMRI)观察一氧化碳中毒后迟发性脑病(DEACMP)患者大脑度中心度(DC)值的变化,推测DEACMP相关脑损伤机制。方法对25例DEACMP患者和25例性别、年龄匹配的健康对照者进行头部rs-fMRI扫描,计算各组全脑DC值图,采用两独立样本t检验进行组间比较(P<0.001,Alphasim校正,体素>30),探讨差异脑区与患者认知功能障碍相关性。结果与健康对照组对比,DEACMP患者右侧额上回、右侧中央前回、右侧角回、右侧顶叶缘上回、左侧顶叶缘上回、右侧海马、左侧丘脑DC值减低;右侧额下回、右侧扣带回、左侧颞上回、左侧颞中回、右侧枕叶舌回、右侧小脑后叶、左侧脑桥、左侧中脑DC值增高。结论脑网络重要节点异常可能为一氧化碳中毒后进展为迟发性脑病的神经病理机制,为一氧化碳中毒后预测迟发性脑病提供一个新的视角。Objective To apply resting state functional magnetic resonance imaging(rs-fMRI)to observe the changes of brain degree centrality(DC)in patients with delayed encephalopathy(DEACMP)after carbon monoxide poisoning,and to infer the mechanism of DEACMP-related brain damage.Methods Head rs-fMRI scans were performed on 25 DEACMP patients and 25 gender-and age-matched healthy controls.The whole brain DC value maps of each group were calculated.Two independent sample t-tests were used to compare between groups(P<0.001,Alphasim)Correction,voxel>30),to explore the correlation between different brain areas and cognitive dysfunction.Results Compared with the healthy control group,the DC values of DEACMP patients’right superior frontal gyrus,right central frontal gyrus,right cingulate gyrus,right superior parietal gyrus,left superior parietal gyrus,right hippocampus,and left thalamus were decreased;the right inferior frontal gyrus,right cingulate gyrus,left superior temporal gyrus,left middle temporal gyrus,right occipital lingual gyrus,right posterior cerebellum,left pons,and left midbrain DC values were increased.Conclusion The abnormality of important nodes in the brain network may be the neuropathological mechanism of the development of delayed encephalopathy after carbon monoxide poisoning,and provide a new perspective for predicting delayed encephalopathy after carbon monoxide poisoning.
