摘　　要：目的探讨机械性血栓清除术对合并溶栓禁忌证急性下肢深静脉血栓形成(LEDVT)患者近期疗效的影响,为后续临床治疗工作提供更多参考。方法回顾性分析急性LEDVT患者共46例临床资料,其中31例无溶栓禁忌症采用导管接触性溶栓治疗设为对照组,15例合并溶栓禁忌证采用机械性血栓清除术治疗设为观察组,比较两组手术情况、手术前后双侧下肢周径、血栓溶解率、肿胀消退率及随访复发率。结果两组术后双侧下肢周径显著小于术前(P<0.05);两组术后双侧下肢周径组间比较差异无显著性(P>0.05);两组血栓溶解率和肿胀消退率比较差异无显著性(P>0.05);同时两组随访复发率比较差异无显著性(P>0.05)。结论机械性血栓清除术治疗合并溶栓禁忌证LEDVT可获得满意近期疗效,具有临床应用价值,但远期预后评价有待更深入研究确证。Objective To investigate the impact of mechanical thrombectomy on the short-term curative effect of patients with acute deep vein thrombosis(LEDVT)combined with contraindications to thrombolysis,so as to provide more reference for subsequent clinical treatment.Methods The clinical data of 46 patients with acute LEDVT admitted to our hospital from January 2016 to December 2018 were retrospectively analyzed.Among them,31 cases without thrombolysis contraindications were treated with direct catheter thrombolysis as the control group,and 15 cases combined with contraindications to thrombolysis were treated with mechanical thrombectomy as the observation group.The operation conditions,bilateral lower limb circumferences before and after the operation,thrombolysis rate,swelling regression rate,and follow-up recurrence rate were compared between the two groups.Results The postoperative bilateral lower limb circumferences of the two groups were significantly smaller than before the operation(P<0.05);there was no significant difference between the two groups of postoperative bilateral lower limb circumferences(P>0.05);the thrombolysis rate and swelling subsided in the two groups There was no significant difference in the rates(P>0.05);at the same time,there was no significant difference in the follow-up recurrence rate between the two groups(P>0.05).Conclusion Mechanical thrombectomy for the treatment of LEDVT combined with thrombolytic contraindications can achieve satisfactory short-term curative effects and has clinical application value.However,evaluation of the long-term prognosis needs to be studied further and confirmed.
