摘　　要：目的总结介入治疗肾动静脉分流的疗效及治疗策略。方法回顾性分析33例肾动静脉分流患者的临床资料,分析总结其介入治疗的安全性、有效性及治疗策略。结果保守治疗3例,介入治疗30例。共行介入手术32次,3例手术失败,技术成功率90.6%。患者症状均于术后3天内消失,围手术期未发生严重并发症,即时临床成功率93.7%。平均随访33.9个月,保守治疗组中,1例随访3年时出现症状后成功行介入治疗;介入治疗组中,1例于术后1个月血尿复发再次行介入治疗失败,余无异常,综合临床成功率为90.6%。结论肾动静脉分流介入治疗安全、有效,应被视为一线治疗手段。Objective To evaluate the efficacy of interventional treatment of renal arteriovenous shunts(RAVS)and to study the therapeutic strategy.Methods Clinical data of patients with RAVS admitted were retrospectively analyzed,the safety and effectiveness of interventional therapy and the therapeutic strategy were studied.Results A total of 33 cases were collected,3 cases were treated conservatively and 30 cases were treated by interventional therapy.In total,32 interventional operations were performed,and 3 failed.The technical success rate was 90.6%.The symptoms disappeared within 3 days after operation.No serious complications occurred during the perioperative period.The immediate clinical success rate was 93.7%.The average follow-up period was 33.9 months.In the conservative treatment group,1 patient developed symptoms at 3-year follow-up and underwent interventional therapy successfully.In the interventional treatment group,1 patient had recurrence of hematuria 1 month after operation and failed to perform re-interventional therapy.The overall clinical success rate was 90.6%.Conclusion Interventional therapy for renal arteriovenous shunts is safe and effective,and should be considered as a first-line treatment.
