改良注水法在老年患者清醒结肠镜检查中的应用

摘　　要：[目的]通过改良注水法、全结肠注水法与传统注气法进镜的对比研究,探讨改良注水结肠镜在老年患者结肠镜检查中的应用优势。[方法]将120例行清醒结肠镜检查的老年患者随机分为3组:全结肠注水组、改良注水组、注气组,每组各40例,比较各组达盲率、达盲时间、辅助腹部按压率、息肉检出率、腹痛评分。[结果]与注气组相比,注水组、改良注水组辅助腹部按压率低、息肉检出率高、腹痛评分明显降低,改良注水组较全结肠注水组、注气组达盲时间明显缩短,均差异有统计学意义(P<0.05);3组达盲率比较差异无统计学意义(P>0.05)。[结论]注水进镜能显著提高患者结肠镜检查的舒适度、增加结直肠息肉的检出率,改良注水法显著缩短进镜时间、提高肠镜操作效率,值得在老年人群结肠镜检查中推广应用。[Objective]To evaluate the advantages of modified water infusion in the colonoscopy of elderly patients by comparing the modified water infusion,the full colon water infusion and the traditional air infusion.[Methods]One hundred and twenty elderly patients who underwent awake colonoscopy were randomly divided into 3 groups:full colon water infusion group,modified water infusion group and air infusion group,40 in every group.The rate of total colon examination,time of reaching the caecum,rate of assisted abdominal compression,rate of polyp detection and abdominal pain scores were compared.[Results]Compared with the air infusion group,the assisted abdominal compression rate and abdominal pain scores of the water infusion group and the modified water infusion group were significantly lower,the polyp detection rate was higher(P<0.05).The modified water infusion group spent less time to reach the caecum than total colon water infusion group and air infusion group,and the difference was statistically significant(P<0.05).There was no significant difference in total colon examination rate among three groups(P>0.05).[Conclution]Water infusion colonoscopy can significantly improve the comfort feeling of colonoscopy and increase the rate of polyp detection.Modified water infusion method can significantly shorten the scope-forward time and improve the operating efficiency of colonoscopy,which is worth popularizing in elderly patients.
