摘　　要：目的探讨术中应用右美托咪啶(DEX)对腹腔镜下全子宫切除患者术后早期胃肠动力的影响。方法选择2017年5月—2019年10月本院收治的60例腹腔镜下全子宫切除术患者作为研究对象,按电脑随机分为右美托咪定组(DEX组,n=30)和生理盐水组(CON组,n=30)两组。DEX组在麻醉诱导前泵注DEX(1μg/kg),术中注入DEX(0.3μg/kg/h)。记录术后第一次排气(FFL)、第一次排便(FFE)时间及首次正常饮食时间。检测术前及术后6小时血清D-乳酸含量。结果与CON组比较,DEX组FFL、FFE及开始正常饮食时间,术后住院时间均明显缩短有统计学意义(P<0.05),术后6 h的血清D-乳酸含量较术前6 h明显升高(P<0.05);与CON组比较,DEX组术后6小时的血清D-乳酸(D-LA)含量明显降低(P<0.05),并且DEX组血流动力学更稳定。结论右美托咪啶有助于腹腔镜下全子宫切除患者术后早期胃肠功能恢复。Objective To investigate the effect of dexmedetomidine(DEX)on postoperative gastrointestinal motility of patients underwent laparoscopic hysterectomy.Methods Sixty patients underwent laparoscopic hysterectomy those admitted to our hospital from May 2017 to October 2019 were enrolled as study objects and they were randomly divided into dexmedetomidine group(DEX group,n=30)and normal saline group(CON group,n=30).In Dex group,DEX(1μg/kg)was pumped before anesthesia induction and DEX(0.3μg/kg/h)was injected during operation.The time of first flatus(FFL),first feces(FFE)and first normal diet were recorded.Serum D-lactate was measured before operation and 6 hours after operation.Results Compared with Con group,the time of FFL,FFE and the time of starting normal diet in Dex group were significantly shortened(P<0.05).The serum D-lactate level was significantly higher at 6 h after operation than that at 6 h before operation(P<0.05).Compared with Con group,the level of D-lactate(d-la)in Dex group was significantly lower(P<0.05),and the hemodynamics of DEX group was more stable.Conclusions Dexmedetomidine is helpful for the early recovery of gastrointestinal function after laparoscopic hysterectomy.
