摘　　要：目的探讨一次性导尿管联合间苯三酚在宫腔镜术前宫颈预处理的临床效果。方法将2018年3月至2020年1月赣州市立医院收治的需行宫腔镜手术患者40例临床资料进行前瞻性分析,依据随机数表法分为对照组和观察组,每组20例。对照组接受宫腔镜术前12 h于阴道后穹隆放置卡孕栓进行宫颈预处理,于术前30 min静脉注射间苯三酚协助宫颈管扩张;观察组行宫腔镜术前12~15 h使用一次性导尿管插入宫颈管进行扩张处理,置入导尿管后阴道塞入两块纱布预防导尿管脱落,于术前30 min静脉注射间苯三酚协助宫颈管扩张。对比两组宫颈软化程度及扩宫效果,并记录两组患者的扩宫时间、手术时间及术中出血量,同时观察两组不良反应发生情况。结果观察组的宫颈软化程度优于对照组(Z=2.995,P=0.003);观察组的宫颈扩张时间(15.56±3.14)s短于对照组(25.54±3.59)s(t=4.653,P<0.001);手术时间(29.24±6.32)min短于对照组(37.36±6.48)min(t=4.012,P<0.001),术中出血量(5.69±4.03)ml低于对照组(14.82±5.49)ml(t=5.995,P<0.001);观察组不良反应发生率5.00%(1/20)略低于对照组30.00%(6/20)(χ2=4.329,P=0.037)。结论针对宫腔镜手术患者使用一次性导尿管联合间苯三酚进行宫颈预处理,可充分软化宫颈,缩短手术时间,降低术中出血量,降低手术不良反应的发生率。Objective To explore the clinical effect of disposable urinary catheter combined with phloroglucinol in cervical pretreatment before hysteroscopy.Methods From March 2018 to January 2020,40 clinical data of patients with hysteroscopy in Ganzhou Municipal Hospital were prospectively analyzed,and they were randomly divided into control group and observation group,with 20 cases in each group.In the control group,carboprost suppository was placed in the posterior fornix of vagina 12 hours before hysteroscopy for cervical pretreatment,phloroglucinol was injected intravenously 30 minutes before operation to assist cervical dilatation;in the observation group,disposable catheter was inserted into the cervical tube 12-15 hours before hysteroscopy for dilation,and two gauzes were inserted into the vagina to prevent the catheter from falling off Phloroglucinol was injected intravenously to assist cervical dilatation.The degree of cervical softening and the effect of uterine dilatation were compared between the two groups,and the uterine dilatation time,operation time and intraoperative blood loss of the two groups were recorded.Results The degree of cervical softening in the observation group was better than that in the control group(Z=2.995,P=0.003);the cervical dilation time(15.56±3.14)s in the observation group was shorter than that in the control group(25.54±3.59)s(t=4.653,P<0.001);the operation time(29.24±6.32)min is shorter than the control group(37.36±6.48)min(t=4.012,P<0.001),the intraoperative blood loss(5.69±4.03)mL is lower than the control group(14.82±5.49)mL(t=5.995,P<0.001);the incidence of adverse reactions in the observation group was 5.00%(1/20)slightly lower than the control group 30.00%(6/20)(χ2=4.329,P=0.037).Conclusion For patients undergoing hysteroscopic surgery,the use of disposable catheter combined with phloroglucinol for cervical pretreatment can fully soften the cervix,shorten the operation time,reduce intraoperative blood loss and reduce the incidence of adverse reactions.
